What is UI-2.1 UIF unemployment benefits claim?

The UI-2.1 is the Unemployment Insurance Fund's Application for Unemployment Benefits in terms of section 17(1) of the Unemployment Insurance Act, read with regulation 3(1). It is the form a former employee completes to claim unemployment benefits after their employment ends, and it also covers the further requirements for reduced work time under section 12(1B).

The one-page form asks for your identity details, contact details and education, confirms whether you are registered as a work seeker and available for work, and ends with a declaration: you undertake to tell the claims officer as soon as you are re-employed or on full pay, and to refund any overpayment. The shaded strip at the bottom is completed by the Department's claims officer.

Who needs to fill out UI-2.1 UIF unemployment benefits claim?

  • Employees who have been dismissed, retrenched or whose fixed-term contract has ended and who contributed to the UIF.
  • Domestic workers who lost their job, including when the employer died.
  • Workers whose hours were cut, claiming reduced work time benefits under section 12(1B).
  • People who claimed before and are applying again — the form asks for the name and ID number of the previous application.
  • Anyone who prefers to prepare the paper form before visiting a labour centre rather than claiming online.

  • Once your employment has ended and you are unemployed.

How to fill out UI-2.1 UIF unemployment benefits claim, step by step

1 Identity number, date of birth and gender

Enter your 13-digit bar-coded identity document number (or passport number) in the boxes, your date of birth as dd/mm/yy and your gender. 2 Names, addresses and contact details

Write your first names and surname as in your ID, then your postal address with code, residential address with code, telephone numbers with dialling codes, occupation, e-mail and fax if you have one.

3 Education

Tick the box for your highest level of education: special school certificate, below grade 8, grade 8–9, grade 10–11, grade 12 or above grade 12.

4 Details of previous application

If you have applied to the UIF before, give the name and ID or passport number under which you applied. The form reminds you to use the separate UI-2.8 form for your banking details.

5 Further requirements

Answer Yes or No: are you registered as a work seeker with a labour centre, and are you capable of and available for work? If you are not capable of or available for work, question 3 asks you to explain. Sign on the line under question 3.

6 Further requirements for reduced work time (section 12(1B))

Only if you claim because your hours were reduced: say whether you are currently employed, whether you are or were on reduced work time (with the date or period on the line), and whether your employer has completed a UI-2.7.

7 Important: declaration

You declare that you are or were unemployed or working reduced hours, that you will inform the claims officer as soon as you are re-employed or receiving full or normal pay, and that you understand failure to do so is fraud and that you will refund any overpayment. Sign as applicant and date it. The shaded boxes below are for the claims officer.

Common mistakes to avoid

  • An ID number that doesn't match the date of birth — FileIt checks both before printing.
  • Leaving the banking details off: they go on the separate UI-2.8, not on this form.
  • Answering 'No' to being available for work without an explanation in question 3.
  • Filling in the reduced work time block when you are claiming ordinary unemployment benefits.
  • Forgetting one of the two applicant signatures.
  • Not telling the claims officer when you start a new job — the form warns this is fraud and overpayments must be refunded.
