What is UI-2.2 UIF illness benefits claim?

The UI-2.2 is the Unemployment Insurance Fund's Application for Illness Benefits in terms of section 22(1) of the Unemployment Insurance Act, as amended by the UI Amendment Act 10 of 2016. A contributor who is unable to work because of illness uses it to claim illness benefits from the Fund.

The one-page landscape form has three parts that matter to you. You fill in your identity and contact details, your education, any previous application under a different ID or passport number, whether you are still employed and, if so, the date you returned to work. A registered medical practitioner completes the medical certificate on the right: their qualifications and registration number, your condition and the period you could not work, with their signature and practice stamp. You then sign the declaration: you will tell the claims officer as soon as you are re-employed and you will refund any overpayment. The shaded strip at the bottom is for the Department's official.

Who needs to fill out UI-2.2 UIF illness benefits claim?

  • Employees who contribute to the UIF and are off work because of illness.
  • Employees still employed but not paid, or paid less, while ill (the employer also completes a UI-2.7).
  • Domestic workers who fall ill and lose income.
  • A proxy appointed by a doctor or legal representative to claim for someone too ill to apply, with proof of the appointment attached.

  • When illness has kept you from working and your pay has stopped or dropped.
  • Before your doctor's appointment, so the doctor only has to complete the medical certificate.
  • Before you go to the labour centre, so you arrive with the form completed.

How to fill out UI-2.2 UIF illness benefits claim, step by step

1 Identity and contact details

Enter your 13-digit bar-coded ID number (or passport or permit number), date of birth as dd / mm / yy, gender, first names and surname. Then your postal and residential addresses with their postal codes and telephone numbers, occupation, e-mail and fax.

2 Education and previous application

Mark your highest level of education: special school certificate, below grade 8, grade 8 to 9, grade 10 to 11, grade 12 or above grade 12. If you applied before under a different ID, passport or permit number, give the name and number you used.

3 Are you still employed?

Mark Yes or No. If you are still employed, the form says your employer must also complete form UI-2.7. If you have returned to work, give the date.

4 Medical certificate

The registered medical practitioner completes this part: their name, qualification and registration number, what you are suffering from and the dates you were not capable of performing work. They sign and date it and add their practice stamp if available. FileIt leaves this part blank.

5 Declaration and signature

Read the 'Important' section: you undertake to tell the claims officer as soon as you are re-employed, and to refund the full amount of any overpayment. Sign by hand at SIGNATURE OF APPLICANT / PROXY and date it. Where a proxy was appointed by a doctor or legal representative, attach proof.

Common mistakes to avoid

  • Lodging the form without the doctor's completed and stamped medical certificate.
  • Forgetting the UI-2.7 from your employer when you are still employed.
  • A date of birth that doesn't match the first six digits of your ID number. FileIt checks this, and the ID number's check digit.
  • Not telling the claims officer when you go back to work, which the form says constitutes fraud.
  • Signing as a proxy without attaching proof of the appointment.
