What is SF 2800?
Standard Form 2800 is the application for benefits payable under the Civil Service Retirement System (CSRS) because of the death of an employee, former employee or retiree who was covered by CSRS. The widow(er), a former spouse, a designated beneficiary, children or their guardian, a parent or the court-appointed executor of the estate use it to claim a survivor annuity or a lump-sum payment.
It asks about the deceased (including former marriages and workers' compensation), about you and how you are applying, about the spouse or former spouse, dependent children, other heirs, the estate, active military service and how you want to be paid. The current edition is Rev. December 2008 (OMB 3206-0156). For a death under the Federal Employees Retirement System, use SF 3104 instead.
FileIt pre-fills your name and date of birth, shows only the sections that apply to you, prints "None" for no former marriages, and fills OPM's own PDF. It checks dates and that every answer fits its box, and saves the finished form in your vault folder. The application must be signed in ink, so you print it, sign it and send it yourself.

Common mistakes to avoid
- Leaving out former marriages: write "none" if there were none, and attach the documents that ended any that existed.
- Forgetting the certified copy of the death certificate, which is required in all cases.
- Applying as executor on the strength of a will: OPM pays only a court-appointed executor or administrator.
- Not returning checks issued to the deceased after the date of death: uncashed checks must be returned to the Treasury.
- Sending it to the wrong office: an employee's agency, or OPM for former employees and retirees.
After you fill it out
Print the form, sign it in ink and attach the documents in the Section K checklist.
Send it to the deceased's employing agency (if an employee at death) or to OPM, P.O. Box 45, Boyers, PA 16017-0045.
If your address changes before you get a claim number, write to OPM with your and the deceased's names, dates of birth and Social Security numbers.
Keep a copy. FileIt saves the PDF in your vault folder.
