What is Arizona Form A-4P?
Form A-4P, the Annuitant's Request for Voluntary Arizona Income Tax Withholding, is how retirees ask the payer of a pension or annuity, or a retirement account administrator, to withhold Arizona income tax. It's issued by the Arizona Department of Revenue, and the current file is the 2026 edition. Withholding from retirement income is voluntary in Arizona: it starts only when you ask for it.
You check box 1 and choose one percentage of the taxable amount (0.5%, 1.0%, 1.5%, 2.0%, 2.5%, 3.0% or 3.5%), and can add an extra dollar amount per payment. Or you check box 2 to end an election you made before. Withholding applies only to the part of a payment that is includable in your Arizona gross income, so not to something like a Roth IRA distribution.
FileIt lets you fill out Form A-4P online in a guided form. It pre-fills your name from the People records in your vault, splits any extra amount into the form's dollars and cents boxes, and prints everything into ADOR's official PDF. The finished form is saved in your vault and can be sent for e-signature. FileIt never submits anything: you give the form to your payer, and the form says not to send it to the Department.

Check box 1 and one percentage: 0.5%, 1.0%, 1.5%, 2.0%, 2.5%, 3.0% or 3.5% of the taxable amount of each payment or distribution. To have more taken out, check the extra-amount box and enter the amount per payment; FileIt prints the dollars and cents in the form's two boxes.

Common mistakes to avoid
- Sending the form to the Arizona Department of Revenue: the form says not to mail or email it there. It goes to your payer.
- Checking more than one percentage: check only one.
- Checking both box 1 and box 2.
- Expecting withholding on a payment that isn't includable in your Arizona gross income, such as a Roth IRA.
After you fill it out
Sign Form A-4P and give it to your pension or annuity payer or retirement account administrator.
The payer withholds Arizona tax until you notify it to change or stop, and sends you a year-end statement of your payments and the Arizona tax withheld.
FileIt saves the finished PDF in the person's folder in your vault.
