What is LPA006 Donor's objection to LPA registration?

If you made a lasting power of attorney (LPA) in England and Wales and someone, usually one of your attorneys, has applied to register it but you don't want it registered, form LPA006 is how you object. You complete it, sign it and send it to the Office of the Public Guardian (OPG). One form covers up to two LPAs, for example your property and financial affairs LPA and your health and welfare LPA.

The form asks for your details and date of birth, the case number (if you know it) and type of each LPA, your reasons, and the names and addresses of the attorneys if you know them. The form warns that OPG will suspend the LPA when it receives it, and your attorneys won't be able to use it unless they apply to the Court of Protection and the court orders it to be registered.

The full name and address of each attorney, if known. There's room for four on the form and four more on the continuation sheet, which you only send if you've used it.

5 Sign and date

Sign the printed form yourself to confirm your objection, add the date, and post it to OPG at the address on page 2.

Common mistakes to avoid

  • Someone other than the donor using this form. Attorneys and people to notify have their own forms.
  • Not signing the form.
  • Objecting to more than two LPAs on one form.
  • Sending the continuation sheet when it's blank. The form says to send it only if you used it.
