What is LP2 Register an LPA?
A lasting power of attorney (LPA) can't be used until the Office of the Public Guardian (OPG) has registered it. Form LP2 is the registration application for LPAs made on the older paper forms: LPA114 (health and welfare), LPA117 (property and financial affairs), LP PW (personal welfare) and LP PA (property and affairs). The donor who made the LPA, or the attorney(s), fill it in and send it to OPG with the LPA itself.
LP2 isn't for LPAs made on the current LP1F or LP1H forms. They are registered through sections 12 to 15 of the same form, and LPAs made online are registered through the online service. GOV.UK also says the older forms can only be registered if they were signed and dated correctly in time: LP PA or LP PW before 1 April 2011, and LPA114 or LPA117 before 1 January 2016. Otherwise a new LPA has to be made.

Who needs to fill out LP2 Register an LPA?
- A donor who made an LPA on one of the older paper forms and wants it registered now, while any mistakes can still be put right.
- Attorneys named in an older LPA who need to register it so they can start acting for the donor.
- Families with an unregistered LPA found among a parent's papers. The attorneys can apply to register it, as long as it was signed in time.
- Not for LPAs made on LP1F or LP1H, or online. Those have their own registration route.
When to use LP2 Register an LPA
- Before the LPA is needed. It can't be used until OPG has registered it.
- Ideally while the donor still has mental capacity. The LP13 guide explains that mistakes can only be corrected then, and an LPA with mistakes can't be registered.
- After you've sent form LP3 to every 'person to be told' (or 'named person') in the LPA. OPG then has to wait four weeks before it can register, so those people have a chance to object.
What you need before you start
- The original LPA, with all its pages, including blank ones.
- The donor's name exactly as it appears on the LPA, even if there's an error in it.
- The names and dates of birth of each attorney applying, exactly as on the LPA.
- Who should receive the registered LPA, and their address if it isn't already correct on the LPA.
- How you'll pay: a phone number for a card payment, or a cheque payable to 'Office of the Public Guardian' with the donor's name on the back.
- For a reduced fee: form LPA120 and recent proof of the donor's low income or benefits.
- Up-to-date addresses for any donor or attorney who has moved, and all the attorneys' addresses if the LPA was made before 1 October 2009.

How to fill out LP2 Register an LPA, step by step
1 Section 1: About the lasting power of attorney
Write the donor's title, first names and last name exactly as they appear on the LPA, then tick whether it's a property and financial affairs LPA or a health and welfare LPA. If you're registering both, fill in a separate LP2 for each.
2 Section 2: The applicant
Tick whether the donor or the attorney(s) are applying. They can't apply together. With more than one attorney, one can usually apply alone. But all attorneys must apply together if they were appointed jointly, or if the LPA says they must all apply. Give each applying attorney's name and date of birth exactly as on the LPA. The form has room for four.
3 Section 3: Who do you want to receive the LPA?
Choose one person OPG can contact with questions: the donor, an attorney or someone else. That person also receives the registered LPA. OPG already has the donor's and attorneys' addresses from the LPA, so only give an address if it has changed, or for someone else. Then tick how they prefer to be contacted (post, phone, email) and whether OPG should write in Welsh.
4 Section 4: Application fee
Choose card or cheque. For a card payment, don't write card details on the form. Give a phone number and OPG will call to take the payment. If the donor has a low income, tick the reduced-fee box and send form LPA120 with proof. The LP13 guide says that if you want the LPA registered quickly and don't have the proof yet, you can pay in full and apply for a refund within three months.
5 Section 5: Signature
Everyone applying signs and dates section 5. By signing you confirm that you're applying to register the LPA, that you've told any people to notify, and that the information is correct. Jointly appointed attorneys all sign. If more than four need to sign, copy the page.
6 Section 6: Addresses
Only needed if the donor or an attorney has moved since the LPA was made, or if the LPA was made before 1 October 2009, when all the attorneys' names and addresses must be given. The LPA was made on the latest date it was signed.
Common mistakes to avoid
- Using LP2 for an LPA made on LP1F or LP1H. Those are registered through sections 12 to 15 of the LPA form itself.
- Correcting a name that's wrong on the LPA. Write the donor's and attorneys' names exactly as they appear on it.
- The donor and attorneys applying together. It's one or the other.
- Only one jointly appointed attorney applying. They must all apply and sign.
- Not sending form LP3 to the people to notify before signing section 5.
- Asking for a reduced fee without recent proof of the donor's low income. The guide says bank statements alone aren't accepted.
- Leaving pages of the LPA out of the envelope, even blank ones.
After you fill it out
