What is Cerfa 14011 — declaration of a lost ID card or passport?

Cerfa 14011*02 is the Ministry of the Interior's declaration of loss (déclaration de perte) for a French national identity card (carte nationale d'identité, CNI) or passport. It records who holds the document, the document's number and issue details, and when, where and how it went missing. The red box at the top of the form warns that declaring a loss or theft makes the document permanently invalid. If you find it again, you must hand it back to the issuing authority and never use it.

Service-Public.fr explains that when you apply straight away for a new identity card in France, the declaration of loss is made at the town hall (mairie) desk when you hand in your application, either on the spot or with this cerfa already filled in, signed and printed. If you don't want a new card straight away, you declare the loss at a police station or gendarmerie instead. Abroad, it is handled by the French embassy or consulate. A theft is declared to the police or gendarmerie, or online, and that declaration is used instead of this form.

1 Which document

Common mistakes to avoid

  • Using this form for a theft. Thefts are declared to the police or gendarmerie.
  • Using the document after declaring it lost. It is permanently invalid.
  • Not giving the names exactly as on the civil-status record, including accents and every first name in order.
  • Ticking 'Personne majeure' for a child. For a minor, a parent or legal representative signs.
  • Forgetting to sign. The signature space has no box, so it's easy to miss.
  • Giving false information. The form points out that a false declaration is punishable under articles 441-6 and 441-7 of the penal code.
