What is TA.1 (Social Security number / registration)?

Modelo TA.1 (Solicitud de afiliación a la Seguridad Social, asignación de número de Seguridad Social y variación de datos) is the application form of Spain's General Treasury of Social Security, the Tesorería General de la Seguridad Social (TGSS). It is used to be registered with Social Security for the first time (afiliación), to be assigned a Social Security number, or to change the personal details Social Security holds about you.

The form asks for your name, sex, identity document, date and place of birth, parents' first names, nationality, address in Spain and contact details, then which of the three requests you are making and which documents you enclose. Its instructions say it must be typed or written in capital letters, without corrections, and that EU, EEA and Swiss citizens registered with a passport must provide their NIE within three months of entering Spain.

FileIt explains each box in English with the Spanish terms alongside, checks your DNI or NIE letter and your Social Security number's control digits, and prints your answers onto the official PDF in capitals, including the receipt on page 3 for the office to stamp. The finished PDF is saved in your FileIt vault. FileIt does not submit anything.

How to fill out TA.1 (Social Security number / registration), step by step

1 1. Datos del solicitante (your details)

Both surnames and your first name, sex (H or M), the type and number of your identity document (with a foreigner's card, the NIE), your Social Security number if you have one, date of birth, parents' first names, place, province and country of birth, degree of disability if any, nationality and, for nationals of other EU countries where it applies, a maiden surname.

2 1.8 Domicilio (address)

Street type (calle, plaza...), the full street name without abbreviations, block, number, staircase, floor, door, postcode, municipality and province.

3 1.9 Datos telemáticos (electronic contact)

E-mail and mobile phone, and whether you accept information messages from Social Security. The instructions note that giving these details means accepting those messages.

4 2. Datos relativos a la solicitud (your request)

Mark one: registration with Social Security, assignment of a Social Security number, or change of details. For a change, give the reason briefly and fill in only the changed boxes plus your name, identity document and Social Security number. Then list the documents you enclose.

5 3. Notificación (notifications)

Choose whether letters go to your address in part 1 or to a different address, which you then give in full.

6 Place, date and signature

Write the place and date and sign. There is a second box for the employer's place, date and signature when applicable. At the foot, name the TGSS provincial directorate or administration office the application is addressed to.

Common mistakes to avoid

  • Writing in lower case or with corrections: the instructions ask for typing or capital letters without corrections or crossings-out.
  • Using a passport when you are not an EU, EEA or Swiss national; others use a foreigner's card with their NIE.
  • Forgetting to provide your NIE within three months when you registered with a passport.
  • Applying for a change of details without your Social Security number, or filling in every box instead of only the changed ones.
  • Not bringing your identity document, or the disability certificate if you gave a degree of disability.

After you fill it out

Print the form, sign it, and hand it in with your documents at a TGSS office. The office stamps the receipt on page 3 and gives it back to you. FileIt does not submit anything.

Keep the stamped receipt and, once you receive it, your Social Security number in your FileIt vault; employers, health services and benefits offices will ask for it.
