What is ISP-2502?
ISP-2502, Consent for Service Canada to Collect Personal Information, is the Service Canada form on which you give, or refuse, consent for Service Canada to collect information about you from third parties: your medical conditions, functional limitations, employment, and volunteer and educational activities. The current edition is SC ISP-2502 (2026-07-01) E.
Service Canada uses the information to decide whether you qualify, or continue to qualify, for Canada Pension Plan (CPP) disability benefits, and it may also use it to assess incapacity under the CPP or the Old Age Security Act, including during appeals before the Social Security Tribunal. The form lists who may be contacted, from doctors, nurse practitioners and hospitals to employers, insurers, workers' compensation boards and government departments. A signed consent is valid for up to 5 years unless you cancel it in writing.

- People already receiving CPP disability benefits whose eligibility Service Canada is reviewing.
- Legal representatives signing for an applicant.
- People asked for information to assess incapacity under the CPP or the OAS Act.
When to use ISP-2502
- With your CPP disability application, or when Service Canada asks for it.
- Again after 5 years, or if you cancelled an earlier consent and want to give it again.
- Choose one option in Section 2; the form warns that not choosing could delay your application or benefit amounts.

1 Section 1 – Information about you
An optional salutation, your SIN, your first, middle and last names, mailing address and telephone numbers.
2 Section 2 – Give or do not give consent
Tick one box. If you don't consent, Service Canada decides on the information in your file, may stop benefits you are receiving, and can require you to provide the information. Then sign and date it, as the applicant or legal representative.
3 Witness
Only if the applicant signs with a mark such as X: the witness reads the section to the applicant, prints their name and telephone number, and signs and dates it.
Common mistakes to avoid
- Leaving both consent boxes empty, which can delay the decision.
- Forgetting the SIN, which also goes at the top of page 3.
- Not giving a copy to your physician or nurse practitioner when they fill out the medical report.
- Having the witness section completed when the applicant signed normally; it is only for a signature made with a mark.
After you fill it out
