What is the yoga teacher training application form template?
A teacher training group works best when the school knows its trainees before day one: how long they've practised, what styles they know, what injuries they're working with and why they want to teach. Collecting that in emails and phone calls makes applications hard to compare and easy to lose.
This template asks for it in four pages — the applicant and the course they want, their practice history and any previous training, their motivation and ability to commit to the hours, and finally payment, access needs, an emergency contact and a signed declaration. Follow-up questions appear only when they're needed, such as certificate uploads for applicants who have trained before or a bursary question for those who ask for one.
Every application is saved as a PDF in your FileIt vault, with uploaded certificates alongside it, ready for your selection conversations.

The yoga teacher training application form, page by page
1 About you
Applicants give their name, email, phone, date of birth and address. They choose the training — foundation (200-hour), advanced (300-hour), a specialist module or not sure — a preferred format such as a full-time intensive, weekends or online, the intake they want and how they heard about the course.
2 Your practice
This page asks how long and how often they practise, the styles they know, whether they have a home practice and which teachers or studios they've studied with. "Have you completed any yoga teacher training before?" opens boxes for the course details and a certificate upload on a yes.
Applicants say whether they teach anything now, and a required question asks about injuries, health conditions or physical considerations, with help text explaining it's for adapting the course. A yes asks for details and whether they're pregnant or expect to be.
3 Motivation
"Why do you want to do this training?" is required. Applicants say what they hope to do afterwards — teach full-time, part-time, deepen their own practice or bring yoga to a specific group, which asks which one — and describe their strengths and challenges. "Can you attend all contact hours and complete the home study?" asks which sessions they might miss on a no.
4 Payment & declaration
Applicants choose to pay in full, pay a deposit, use an instalment plan or apply for a bursary, which asks about their circumstances. Access or learning needs and an emergency contact follow, and the form closes with a declaration, signature and date.
Make the template yours
- Replace the course list with your own trainings and intake dates
- Add your fees and refund terms in a paragraph on the payment page
- Remove the bursary option if you don't offer concessions
- Add a question about the lineage or philosophy texts your course is built around
- Set a closing date so the form stops accepting applications when the intake is full
- Route applications into an Admissions folder for each intake
Tips for a better yoga teacher training application form
- Follow up on the physical considerations answer with a conversation rather than a decision on paper
- Read the motivation answers alongside a short call — written answers favour confident writers
- Be clear in your course information about the certification body your training is registered with, if any
- Tell applicants when they'll hear back so they don't chase you
- Keep the bursary process separate and confidential from the main selection
Every response becomes a PDF in your vault
Each application is filed as a PDF in your FileIt vault with any certificate the applicant uploaded, and you're emailed a notification. The applicant can receive a copy of what they submitted.
Use the Responses table to compare applicants by course, format or payment option, and export to a CSV to plan the group.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
Yoga teacher training application form: frequently asked questions

No. They fill it in from a public link on your website or a link you email them.
Can applicants upload certificates from earlier trainings?
Yes. Answering yes to previous training shows an upload for up to two certificate files.
Does the form register trainees with a certifying body?
No. It collects applications for your school. Registration with any certifying body is a separate step your school handles.
Can we take payment through the form?
No. FileIt Forms records the payment option the applicant chooses — you send the invoice or payment link yourself.
Can we stop applications when the course is full?

Why ask about injuries and health?
So teachers can adapt practice and support each trainee. The help text tells applicants it isn't used to turn them away — change it if your policy is different.
