What is the tanning session consent form template?
Tanning salons need different information for different services. Before a sunbed session you need to know the client's skin type, risk factors and anything that makes them sensitive to UV; before a spray tan, allergies, asthma and a patch test matter more.
This template asks which service the client wants and shows the right questions. Sunbed clients see a note about minimum age, a skin type scale (type 1 shows a note suggesting a spray tan instead), risk factors and photosensitising medication, with a note to check with a doctor when something applies. Spray tan clients answer questions on sensitivities, asthma and a patch test. Everyone is asked about previous tanning and reactions. The last page sets out sunbed rules and requires acknowledgements on goggles, session limits and UV risks, or shows spray tan aftercare, then a declaration and signature.
Each consent is saved as a signed PDF in your FileIt vault. Edit the rules to match your local regulations.

For sunbed clients: skin type from 1 to 6, with type 1 showing a note suggesting a spray tan. Risk factors such as skin cancer history, many moles, recent sunburn, pregnancy or photosensitive conditions, and whether medication increases sun sensitivity, with details. Any risk factor shows a note to check with a doctor or pharmacist.
