What is the school counselor referral form template?

Counsellors hear about students in corridors, by email and through notes left on desks. A referral form gives them the same core information every time: who the student is, what has been noticed, how urgent it is, and whether the family can be contacted.

This template has three pages. The referrer says who they are, from teacher to parent or the student themselves, and gives the student's name and grade. The concerns page starts with urgency; choosing immediate risk shows a note to contact the safeguarding lead now. Then areas of concern, what's been noticed, how long it's been going on and a 1-5 impact scale. The last page covers what's been tried, other support, whether the student knows and is willing, and, for staff referrals, whether the family may be contacted. Parents give consent on the form itself.

Each referral is saved as a PDF in your FileIt vault for the counselling team. The form doesn't replace your safeguarding procedures.

The school counselor referral form, page by page

1 You & the student

Your name, whether you're a teacher, other staff, a parent or the student, and your email are required, with an optional phone. Then the student's name and grade, and optionally their class or homeroom.

2 Concerns

How urgent it is: immediate risk shows a note to contact the safeguarding lead and emergency services instead of waiting. Then a checklist of concerns, from anxiety and low mood to bullying, grief, attendance, self-harm or eating, what you've noticed, how long it's been going on and a required impact scale from 1 to 5.

3 Support & contact

What's been tried, other support the student has, and, unless it's a self-referral, whether the student knows about the referral and is willing to attend. Referrers other than parents say whether the counsellor may contact the family, with a box for anything to know first. Parents agree to a consent statement instead. Best times to reach you and a confidentiality note close the form.

Make the template yours

  • Put your safeguarding lead's name and number in the immediate-risk note
  • Edit the concern list to match how your team records referrals
  • Make the form open only to staff by sharing the link internally, and create a separate one for families
  • Add a question for the student's preferred meeting time
  • Route referrals to a restricted counselling folder
  • Adjust the parent consent wording to your school's policy

Tips for a better school counselor referral form

  • Never use a form as the only route for an immediate safeguarding concern
  • Acknowledge every referral so referrers know it was received
  • Check family contact wishes before calling home
  • Review the Responses table weekly for referrals still waiting
  • Have your safeguarding lead approve the wording before you publish

Every response becomes a PDF in your vault

Each referral is saved as a PDF in your vault and the counselling team is emailed. The referrer receives a copy.

The Responses table shows referrals by urgency and concern, so the team can see what needs attention first.

  1. Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
  2. Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
  3. Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
