What is the puppy training registration form template?
Before a puppy joins a group class, a trainer needs to know a handful of things: how old the puppy is, whether their vaccinations are far enough along, whether there are any health or behaviour worries, and what the owner actually wants help with. Gathering that by text message the night before the first class is how vaccination cards get forgotten.
This registration form collects it all upfront across four short pages: the owner and puppy, health and vaccinations with an upload for the vaccination record, behaviour concerns and training goals, and finally the course, preferred times and a class agreement the owner signs.
Each registration is saved as a PDF in your FileIt vault together with the uploaded vaccination record, so you can check it before confirming a place and have it on hand if a question comes up later.

The puppy training registration form, page by page
1 Owner and puppy
The owner gives their name, email, mobile and address, plus any other family members who will attend or train at home. The puppy's section asks for name, breed or mix, approximate date of birth, sex, whether they are spayed or neutered, where they came from (breeder, rescue, friend or family, pet shop) and the date they came home.
2 Health and vaccinations
Owners name their veterinary practice and phone number, tick the vaccinations given so far and enter the date of the most recent one. The vaccination record upload is required, so you can check it before the first class. They also say whether the puppy is microchipped, and answering yes to health issues, allergies or medication opens a box for details.
3 Behaviour and goals
A checklist covers common puppy problems such as toilet training, mouthing, jumping up, lead pulling, chewing, barking, crying when left alone, nervousness, guarding and recall. If the owner says the puppy has growled, snapped or bitten in a way that worried them, they're asked what happened and when. A 1 to 5 scale rates confidence with new people and places, and the owner describes their own experience and what they most want the puppy to learn.
4 Class choice and agreement
Owners pick a course (puppy socialisation, foundation course, adolescent course or one-to-one sessions) and the times that suit them. They say whether food treats can be used, and if not, which foods to avoid. The page ends with the class agreement, an optional photo permission and the owner's signature.
Make the template yours
- Rename the course options to match your own classes and age limits
- Edit the vaccination checklist to match what your vet partners and local rules expect
- Add your course fee and payment instructions to the confirmation message
- Set a maximum number of responses so a course closes when it's full
- Add a question about other dogs at home if you run multi-dog sessions
- File registrations into a vault folder per course or start date
Tips for a better puppy training registration form
- Check the uploaded vaccination record before confirming a place in a group class
- Read the behaviour answers before week one so you can position nervous puppies away from the busiest spot
- Follow up any yes to growling or biting with a phone call, and consider one-to-one sessions first
- Ask owners to bring the treats they listed as safe so there are no surprises in class
- Keep the agreement wording short and plain so owners actually read it
Every response becomes a PDF in your vault
Each registration becomes a signed PDF in your FileIt vault, with the vaccination record attached, and you get an email when a new one comes in. Owners can receive a copy of their answers too.
The Responses table gives you a class list you can filter by course and export to CSV, which is handy for a register or for planning group sizes.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
