What is a scuba diving waiver?

Dive operators take certified divers they've never met to sites they may not know, sometimes hours from a recompression chamber. A waiver can't make that safe on its own, but it does make sure every diver has stated their certification, experience and fitness, and agreed to the rules that keep a dive boat running safely.

This template records the diver's certification level and number, logged dives and date of their last dive, dive insurance and an emergency contact. It ties in a recognised diving medical questionnaire, with the doctor's sign-off when the diver answered yes. You set your maximum depth, the gas reserve for surfacing, an optional time limit and whether a dive computer is required. It covers briefings, the captain's right to cancel or end a dive, buddy rules and a lost-buddy procedure, no-decompression limits and safety stops, overhead environments, marine life and equipment. The risks, including decompression illness and delayed treatment at remote sites, are set out plainly.

Check the certification card at check-in, countersign, and keep the waiver in your FileIt vault.

Tips before you send it

  • Use a recognised diving medical questionnaire with this waiver. The waiver refers to it.
  • Offer a check dive or refresher to divers who haven't dived for a while.
  • Keep oxygen and a written emergency plan on every boat, including the nearest recompression chamber.
  • Your training agency, insurer or local regulator may require their own forms. Use those alongside this one.
