What is a patient discharge letter?

A patient discharge letter, sometimes called a dismissal or termination letter, tells a patient that a doctor, dentist or practice will no longer provide their care. Ending care is sometimes necessary — repeated missed appointments, unpaid balances, abusive behaviour or a breakdown of trust — but it has to be done carefully, so the patient is not left without care and the practice is not accused of abandoning them.

This one-page letter gives the date care ends, says what care remains available until then (urgent and emergency care and appropriate prescription renewals, or care as usual), adds an optional medication or treatment note and a bold reminder about emergency services. It explains how to find a new provider, how to have records transferred and whether copying is free, and any outstanding balance — while stating that the balance doesn't affect care before the end date or the records transfer. The reason is optional, and the letter records how it was sent.

Using the patient discharge letter, step by step

1 Address the letter

Enter the patient's name and address, a parent or guardian for a minor, and the provider's name if only one provider is ending care.

2 Choose the date and transition care

Pick an end date that gives the patient time to find a new provider, and what care stays available until then. Add any medication note.

3 Explain records and any balance

Edit how to find a new provider and request records, choose whether copies are free, and add any outstanding balance and due date.

4 Sign and send

Choose how it is being sent, sign for the practice, and send. Keep the signed copy and delivery proof with the patient's record.

Tips before you send it

  • Check your licensing body's, insurer's and local rules on notice periods and on patients in the middle of treatment before choosing a date.
  • Keep the reason short and factual, or leave it out.
  • Never end care for a reason the law forbids, such as discrimination.
  • Send it in a way you can prove — the letter records the method used.

Patient discharge letter: frequently asked questions

How much notice should a patient get?

Enough to find another provider. Licensing bodies, insurers and local law may set a minimum notice or transition period, so check before you pick the date. The letter promises urgent and emergency care until then.

Do I have to give a reason?

Not usually. The reason is optional in this template; many practices give a brief, factual one or none.

Can I refuse to transfer records because of an unpaid balance?

The letter says the balance doesn't affect care before the end date or the transfer of records. Records access is often protected by law, whatever is owed.

Is a document signed electronically legally binding?

In most countries, including the US (ESIGN Act and UETA), the UK, the EU (eIDAS) and Canada, an electronic signature is generally valid for an ordinary agreement like this one. FileIt keeps an audit trail and a Certificate of Completion with the signed PDF as evidence of who signed and when.

Is this template free?

Yes. The template is free to use with the eSignature app in FileIt.
