What is a holistic therapy consent?
Complementary therapists need clients to understand what a session involves, to tell them about health issues before they start, and to know that the therapy is not a substitute for medical care. Insurers and professional bodies often expect a signed consent and health declaration on file.
This form lets you choose the therapy and describe the session in plain words: positions, clothing, length. You pick the level of physical contact, from no touch to massage of bare skin, and the form states areas that will never be touched and that the client can ask for any area to be avoided. List any essential oils or products so allergies can be flagged. The client declares pregnancy, conditions, medication and allergies, and any doctor's approval. The form explains common after-effects, including for breathwork, makes clear you don't diagnose or advise on medication, and records the client's right to stop at any time. A parent signs for under-18s.

Using the holistic therapy consent, step by step
1 Describe the session
Choose the therapy, describe what happens and how long it lasts, and pick the level of physical contact.
2 List products
Name the oils or products used on the skin or inhaled, so the client can flag allergies, including nut-based carrier oils.
3 Health declaration
The client records pregnancy, conditions, medication and allergies, and any doctor's approval you asked for.
4 Sign before the session
Send it ahead of the first appointment. The client signs, you countersign, and the form is kept with your client notes.
Tips before you send it
- Go through the health declaration in person as well. It's a starting point for a conversation, not a replacement for one.
- Ask clients to update you before every session: new medication, injury or pregnancy changes what is suitable.
- Avoid claims that a therapy treats or cures a condition, in the form and in your marketing.
- Check your insurer's and professional body's requirements for consent records.
- Ask a client to check with their doctor first if they have a serious or unstable condition.

Holistic therapy consent: frequently asked questions
Which therapies does it cover?
Reiki, reflexology, aromatherapy, breathwork, sound healing, or a combination. You describe the session in your own words, so it can be adapted to similar complementary therapies.
Does it say the therapy is not medical treatment?
Yes. The client confirms they understand it is complementary, that you don't diagnose or prescribe, that you won't advise changing medical treatment, and that no result has been promised.
Can clients stop a session?
Yes. The form records the client's right to stop a session or withdraw consent at any time without explanation, and your right to end a session if you're concerned for their wellbeing.
Does it cover children?
Yes. Enter the parent or guardian's name; they sign, and the form records that they will stay at the practice during the session.
Does it limit my liability?
No. It records informed consent and health disclosures. It states that it does not limit liability that cannot be limited by law, such as for negligence causing injury.
