What is Nevada VP-222?

Form VP-222 is the Nevada Department of Motor Vehicles' Application for Vehicle Registration. The DMV's forms page says it's needed when applying for original registration and must be signed by the owner.

The one-page form asks for the VIN, year, make and model, the declared gross weight of a truck or bus or the unladen weight of a trailer, the county where the vehicle will be based, and the odometer reading. Then come the applicant's full legal name, a Nevada driver's license or ID number (or date of birth, FEIN for businesses, or motor carrier number), the physical Nevada address, mailing address, phone and email. By signing, you declare you'll keep the vehicle insured as Nevada law requires; trailers are exempt from the insurance requirement.

FileIt prints your answers into the DMV's PDF, one VIN character per box, and can send it for your e-signature. The PDF is saved in your vault. FileIt doesn't file anything with the Nevada DMV.
