What is Colorado DR 2395?

Form DR 2395 is the Colorado Department of Revenue, Division of Motor Vehicles' Application for Title and/or Registration. You use it to title a vehicle in Colorado, register it, or both.

Page 1 covers the vehicle (VIN, year, make, model, odometer, body, color, empty weight, size, fuel, dealer number, purchase date, MSRP), flex fuel and plug-in electric questions, bus capacity, off-highway use, weights, commercial use with the DOT number and EIN, hazmat, registrant only, the owners or lessor, any lessee, the mailing address and the first lienholder. Page 2 is the signer's certification under penalty of perjury with their secure and verifiable ID, and a witness who affirms the ID was presented. Page 3 asks about the DR 2383 joint tenancy acknowledgment and the Keep Colorado Wild Pass, and has boxes the county or EVTR vendor completes.
