What is Form SSA-7050?

Your free online Social Security statement shows yearly totals. When you need more, such as the names and addresses of the employers who reported your wages, or a certified record for a pension plan or a lawsuit, you order it with Form SSA-7050 and pay a fee.

You choose one kind of information per form: an Itemized Statement of Earnings, which includes periods of employment or self-employment and employers' names and addresses (certification optional), or Certified Yearly Totals of Earnings. You can also ask for a child's, a ward's or a deceased relative's earnings, with proof of your relationship.

FileIt pre-fills the name and date of birth from your vault, checks the years you ask for, works out the amount for the optional credit card section from the fees printed on the form, and prints everything on SSA's own PDF (11-2024 edition), accented names included. It never stores a card number: you write that by hand.

Fees (as printed on the form)
$61.00 itemized statement, $96.00 certified itemized statement, $35.00 certified yearly totals. Not refundable.

When to use Form SSA-7050

  • When yearly totals aren't enough and you need employers' names and addresses, or a certified record.
  • Not for free yearly totals: get those at www.ssa.gov/myaccount.
  • If you believe your earnings are wrong, say so and show the years; the form says there is sometimes no charge in that case.

What you need before you start

  • The name on the most recent Social Security card, the SSN (one per request) and the date of birth, plus the date of death for a deceased person.
  • The years you want, and why you need an itemized statement.

How to fill out Form SSA-7050, step by step

1 1. Whose earnings

Name as on the latest Social Security card, SSN, date of birth (and of death), and other names used.

2 2. What you need

Choose ONE: itemized statement (with the reason and years, certified if you want) or certified yearly totals (with years).

3 3. Someone else

Only if the statement should be sent to another person or organization.

4 4. Signature and payment

Your relationship if you're signing for someone, phone and address; sign by hand. Add the card details on page 4 if paying by card.

Common mistakes to avoid

  • Choosing both kinds of statement on one form: SSA returns it. Use two forms, each with its own payment.
  • Sending it without the right fee, which the form says gets it returned.
  • Not saying why you need an itemized statement: if you don't, the form says SSA charges the fee.
  • Requesting someone else's earnings without proof of relationship, or of death.

After you fill it out

Print the form, sign it by hand, and write the card number and expiration date if you pay by card.

Mail it with the fee and any proof to Social Security Administration, P.O. Box 33011, Baltimore, Maryland 21290-33011, within 120 days of signing.
