What is Form SSA-7008?

Your Social Security benefits are worked out from the earnings on your record, so a missing or wrong year can cost you later. Form SSA-7008 is how you tell the Social Security Administration that you've examined your statement (or record) of earnings and it is not correct, and give the information and evidence it needs to correct it.

Item 7 lists wages from an employer, one entry per calendar period: the years or months, the type of work, the employer's name, address and phone, the correct Social Security (FICA) wages and the evidence you enclose, such as a W-2 or W-2c. Item 8 lists self-employment earnings, one entry per year, and item 9 asks about your tax return for that income.

FileIt pre-fills your name and date of birth from your vault, checks that each entry is complete and that your explanations fit their boxes, and prints everything on SSA's own PDF (09-2023 edition), accented names included. The finished form is saved in your vault folder.

How to fill out Form SSA-7008, step by step

1 About you

Name, date of birth, the name on your card, other names, your SSN, any other SSNs used, and whether SSA may give your name to your employers.

2 Item 7: wages

In date order, each period you believe is wrong, with the employer, the correct wages and the evidence you enclose.

3 Items 8 and 9: self-employment

Each wrong year of self-employment earnings, and whether you filed a return and have a copy.

4 Remarks and signature

Explain anything missing, such as why you have no evidence. Then your mailing address and phones; you sign by hand.

Common mistakes to avoid

  • Listing years that are already correct: the form asks only for periods you believe are wrong.
  • Sending no evidence and no explanation: if you can't send evidence, explain why in item 10.
  • Saying no to letting SSA disclose your name to your employers, which the form says prevents a thorough investigation.
  • Leaving out other names or numbers your wages might have been reported under.

After you fill it out

Print the form and sign it by hand.

Enclose copies of your evidence (W-2s, pay stubs, tax returns).

Mail it to Social Security Administration, 6100 Wabash Ave., Baltimore, Maryland 21215 (page 3 also says you can send or bring it to your local Social Security office), and keep a copy. FileIt files the PDF in your vault folder.
