What is Form MV-912?

Form MV-912 is the New York State Department of Motor Vehicles' Vehicle Bill of Sale. In a private sale, the seller fills it in to state that, for the price shown, they sell, transfer and convey the vehicle to the buyer. It names both people, describes the vehicle by year, make, model and identification number, and has room for any terms of the sale. The current edition is MV-912 (11/21).

The bill of sale is a record between the two of you rather than an application: nothing is filed with it on the spot. The buyer usually keeps it with the signed-over title as proof of the purchase and the price paid, and brings it along when registering and titling the vehicle. The seller keeps a copy for their own records.

FileIt lets you fill out the MV-912 online. It pre-fills the seller's name from the People records in your vault, prints the seller's and buyer's names both in the opening sentence and in their own boxes, puts the VIN one character per box, and wraps any terms of sale across the large box. The filled PDF is saved in your vault, and you can send it to the buyer and seller for e-signature. FileIt never files anything with DMV.

Who needs to fill out Form MV-912?

  • Private sellers of a car, truck, motorcycle, trailer or boat in New York.
  • Buyers who want written proof of the purchase price for registration and sales tax.
  • Family members transferring a vehicle as a gift, who want the price (or a gift) on record.
  • Anyone selling a vehicle whose title doesn't have room to record the price.

When to use Form MV-912

  • At the moment the vehicle and money change hands, so both sides sign the same version.
  • When you agree on terms such as 'sold as is' or a payment plan and want them written down.
  • When the buyer will register the vehicle in New York and needs to show what they paid.

What you need before you start

  • The seller's and buyer's full names and addresses.
  • The agreed price.
  • The vehicle's year, make and model, from the title or registration.
  • The 17-character vehicle identification number (VIN), or a boat's hull identification number.
  • Any terms you both agreed to, such as warranty, payment schedule or what is included.

The 2021 edition has 1 page. FileIt asks for it in 4 parts, and it is signed by Seller and Buyer:

  3. SellerSeller
  4. BuyerBuyer

How to fill out Form MV-912, step by step

1 The opening statement

Enter the seller's name, the price and the buyer's name. Together they read as one sentence: 'I, (seller), in consideration of $(price), do hereby sell, transfer and convey to (buyer) the following vehicle.'

2 Description of vehicle

Give the year, make and model, then the VIN or hull number one character per box. Copy them from the title so they match DMV's records exactly.

3 Terms and conditions

Optional. Use it for anything the two of you agreed, for example 'sold as is, no warranty', a deposit and balance, or extra parts included. FileIt checks that the text fits the box.

4 Seller and buyer boxes

Each box repeats the person's name and adds their address. The form asks you to print clearly; FileIt prints everything except the signatures.

5 Sign and date

The seller and the buyer each sign and date their own box. With FileIt you can send the PDF for e-signature, or print it and sign on paper.

Common mistakes to avoid

  • Copying the VIN from memory or a sticker instead of the title, so one character is wrong.
  • Leaving the price blank. Write the amount actually paid, or 0 for a gift.
  • Using a nickname instead of the name on the title and registration.
  • Signing before the terms are filled in, so the two copies don't match.
  • Treating the bill of sale as the title transfer. The title still has to be signed over separately.

After you fill it out

Both of you sign and date the form, on paper or through FileIt's e-signature.

The seller signs over the title; the buyer takes the title, the bill of sale and proof of insurance to DMV to register the vehicle.

FileIt keeps the signed PDF in your vault, so the seller has a copy if questions come up later.

Form MV-912: frequently asked questions

Does New York require a bill of sale to sell a car?

The title is what transfers ownership. A bill of sale such as MV-912 records the price and the parties, and DMV may ask for proof of the price when the buyer registers the vehicle.

Does Form MV-912 need to be notarized?

The form has no notary section. The seller and the buyer simply sign and date their own boxes.

Can I use MV-912 for a boat?

Yes. Enter the boat's hull identification number in the 'Vehicle or Hull Identification Number' boxes.

What should I write if the vehicle is a gift?

Enter 0 as the price. DMV may ask for more about a gift when the buyer registers it.

Can we sign the bill of sale electronically?

FileIt can send the filled PDF to both of you for e-signature. The form doesn't say whether DMV accepts an e-signed copy, so print and sign on paper if you're unsure.

Can I fill out MV-912 online?

Yes. FileIt fills it out online and prints your answers on the DMV's own PDF. FileIt doesn't file it anywhere: the buyer keeps it and brings it to DMV.
