PennDOT Form MV-63: Pennsylvania Change of Address

What is Form MV-63?

Form MV-63, Change of Address, is how you tell the Pennsylvania Department of Transportation (PennDOT) that you've moved. The form itself says Pennsylvania requires you to report your new address within 15 days of moving. One form covers up to two people with a driver's license or photo ID who live at the same new address, up to three vehicles registered there, and a Person with Disability Parking Placard. The current edition is MV-63 (3-25).

The address change also serves as a request to update your voter registration, unless you tick the opt-out box for that person. If you're not registered to vote, PennDOT's form says you'll receive an application to register. The form can't be used for a name change or for a commercial driver's license (CDL); CDL holders use Form DL-80CD.

Who needs to fill out Form MV-63?

Pennsylvania drivers and photo ID holders who have moved to a new address.
Households where two people with a license or ID card move together — both fit on one form.
Owners of up to three Pennsylvania-registered vehicles that need the registration address changed.
Holders of a Person with Disability Parking Placard whose address has changed.
People changing only a vehicle registration address — the form still asks for your license or photo ID number in Section A.

When to use Form MV-63

Within 15 days after you move to a new address.
When you'd rather mail a paper form than change your address online at pa.gov/dmv.
When you want the move to update your voter registration at the same time (or tick the box so it doesn't).
Not for a name change, and not if you hold a commercial driver's license — CDL holders use Form DL-80CD.
Not for leased vehicles: the leasing company submits Form MV-1L for the registration address change.

What you need before you start

Your Pennsylvania driver's license or photo ID number and date of birth (and the same for a second person at the address).
Your full name as it appears on the license: last, first and middle.
Your new Pennsylvania street address with city and ZIP code. A P.O. box can be added but can't be the only address.
For each vehicle: the title number (including any letters), the make and the registration plate number.
For a disability placard: the placard number and its expiration date.
Form MV-8, "Self Certification for Proof of Residency", if you qualify for one of the out-of-state address exceptions.

How to fill out Form MV-63, step by step

1 Section A — Driver's license or photo ID

Enter the license or photo ID number, date of birth and full name (last, first, middle) for up to two people who live at the same new address. The form's instructions ask you to include your license or ID number here even when you're only changing a vehicle registration address.

2 Section B — Vehicle registration

List up to three vehicles: the title number including its letters, the vehicle make and the registration plate number. If a vehicle is leased, leave it off — the form tells you to contact your leasing company so it can submit Form MV-1L for the registration address change.

3 Section C — New address

Enter your new street address, city, state and ZIP code. The instructions say only a Pennsylvania address may be recorded on title and registration products, and it must be your actual numbered street address. A P.O. box number may be used in addition to the street address, never on its own.

4 Section D — Voter registration update

By default, the change of address also serves as a request to update each applicant's voter registration. Tick the box for applicant #1 or #2 only if you don't want that. If you're not registered, you'll receive an application to register; you must be a U.S. citizen to register to vote in Pennsylvania.

5 Section E — Disability parking placard

Only if you have a Person with Disability Parking Placard that needs the new address: enter the placard number and its expiration date. The form notes you can also change the placard address online on PennDOT's placard website.

6 Out-of-state address exceptions

The instructions list the only cases where motor vehicle documents are processed with an out-of-state address: U.S. Armed Forces personnel or their dependents, federal or state government employees (or their immediate families) working outside Pennsylvania, certain businesses with no Pennsylvania location whose vehicles regularly operate in the state, and park model trailers permanently located in Pennsylvania. In those cases, attach Form MV-8. Mobile home records are exempt from the Pennsylvania address requirement.

Common mistakes to avoid

Waiting more than 15 days after the move to report it.
Giving only a P.O. box — PennDOT needs your actual Pennsylvania street address.
Using the MV-63 for a name change or a CDL; those need a different form (DL-80CD for a CDL).
Listing a leased vehicle instead of asking the leasing company to submit Form MV-1L.
Leaving out the license or photo ID number when changing only a vehicle registration address.
Not noticing that the form updates your voter registration unless you tick the opt-out box.

Mail the form to PennDOT, Bureau of Motor Vehicles, PO Box 68287, Harrisburg, PA 17106-8287 — the address printed at the top of the form. Attach Form MV-8 if you're claiming an out-of-state address exception.

How long do I have to change my address with PennDOT?

The form says Pennsylvania requires you to report your new address to PennDOT within 15 days of moving.

Can I change my address online instead?

Yes. The form points you to pa.gov/dmv to change your address online. The MV-63 is the paper alternative you mail in.

Does the MV-63 update my voter registration?

Yes, unless you tick the box in Section D for that applicant. If you aren't registered, you'll be sent an application to register.

Can two people use one MV-63?

Yes. Section A has room for two customers at the same address, and Section B for up to three vehicles.

Can I use a P.O. box as my address?

Only in addition to your actual street address. PennDOT requires a numbered Pennsylvania street address on title and registration products, with a few listed exceptions that need Form MV-8.

Is there a signature on the MV-63?

No. The current edition, MV-63 (3-25), has no signature or date line.
