Form CPF-CA(C): Close a Child's CPF Account Online

What is Form CPF-CA(C)?

Form CPF-CA(C) is the children's version of the CPF account closure form. When a family gives up Singapore PR or citizenship and moves away, each child who had a CPF account also needs that account closed. The child can't apply for themselves, so a parent or legal guardian fills in and signs this form, proves the relationship, and receives the money into their own bank account. The current edition is Form CPF-CA(C) 09/2026, six pages long.

It differs from the adult Form CPF-CA in a few important ways. There are two sets of particulars — the child's and yours — plus a tick box for whether you are the father, mother or legal guardian. The bank details must be yours, not the child's. The declaration is made on the child's behalf and covers fewer schemes. And the form offers no "Write to Us" upload: it has to be completed at a CPF Service Centre in Singapore, or witnessed and certified overseas at a Singapore Overseas Mission or by a notary with an apostille.

Who needs to fill out Form CPF-CA(C)?

Parents of a child whose Singapore PR has been cancelled or renounced along with the rest of the family's.
Parents of a child who has renounced Singapore citizenship and still has a CPF account.
Legal guardians of such a child, who must also show the guardianship documents or court order.
Families who have already moved abroad and need the form witnessed at a Singapore Overseas Mission or by a notary with an apostille.
Families still in Singapore who can book a CPF Service Centre appointment before leaving.
Not for adults: a member closing their own account uses Form CPF-CA.

When to use Form CPF-CA(C)

Once the child is no longer a Singapore Citizen or Permanent Resident — the form is only for such children, and you declare this when you sign.
Ideally at the same time as closing your own account with Form CPF-CA, so the family's paperwork, documents and witnessing appointment can be handled together.
Before leaving Singapore if possible, when a CPF Service Centre can witness it; from overseas the form must be witnessed and certified at a Mission or by a notary plus apostille.
If the child's MediShield Life cover should continue, contact CPF before you submit — approval of the application ends that cover.

What you need before you start

The child's Singapore NRIC or CPF account number, current passport, and foreign identity card or citizenship certificate (both sides) if there is one.
Proof of your relationship: the child's birth certificate, or legal documentation of guardianship including any relevant court order.
Your own identification documents and your passport or identity card number.
Where available, the documents used during the child's residence in Singapore: NRIC or FIN card, old passports showing the NRIC number, and CPF letters with the child's name and account number.
For a child who was a Citizen or PR, the ICA renunciation letter (not needed for a Malaysian child), and for an ex-PR child the entry permit (Form 5) or re-entry permit (Form 7) where applicable.
A bank letter or statement showing YOUR full name and account number.
For an overseas account: the recipient bank's name, address and SWIFT code, any intermediary bank, and codes such as an IBAN or routing number.
A deed poll or change of name certificate if the child's name has changed.

How to fill out Form CPF-CA(C), step by step

1 1 Child's particulars

The child's name exactly as in their identification document, in block letters; the child's Singapore NRIC or CPF account number in nine boxes; their foreign identification and passport numbers; and their date of birth as DD / MM / YY.

The child's CPF account number is the NRIC or FIN they held in Singapore — it's printed on CPF letters and statements sent for the child.

2 2 Applicant's (parent / legal guardian) particulars

Your own name, identification or passport number, contact number, email and correspondence address, and a tick against Father, Mother or Legal guardian.

The contact number box is short, so keep it to digits, spaces and a leading plus sign. Use a postal address where you'll receive letters after leaving Singapore.

3 3 Bank details (the "Bank Account")

The first note is the one people miss: give the applicant's — your — bank details. The account must not be a trust account. A Singapore account is paid by Interbank GIRO; an overseas account by telegraphic transfer, for which the form also asks for the recipient's account name, bank address, SWIFT code and any intermediary bank.

The notes start on page 1 but the boxes themselves continue at the top of page 2. If your bank doesn't accept Singapore dollars the payment is converted to US dollars, and you bear the bank charges and exchange-rate differences.

4 4 Declaration and agreement

Printed as "mandatory". You confirm you are authorised to apply for the child and that the child is not a Citizen or PR, and you apply to withdraw everything in the child's CPF and CPF Investment Account. If the child later becomes a Citizen or PR, the child would have to refund those amounts with any interest.

The statements then cover the child's CPFIS investments, any property bought with CPF, the ending of MediShield Life cover, reporting unpaid employer contributions, and consent for your bank details to be used to pay you. Sign by hand on page 3, in front of the officer or witness; your ID number and the date print beside the signature box.

5 5 Witness certification

Headed "For Applications Not Completed in CPF Service Centre". Overseas, an official at a Singapore Overseas Mission — or a Notary Public, followed by an apostille from your country's Competent Authority — watches you sign again here and certifies the form and the original documents.

6 6 What to do next

7 Important notes (pages 5 and 6)

Page 5 lists the supporting documents in five groups, starting with proof of relationship between you and the child. Page 6 covers the mode of payment and the service standard: generally 12 weeks from receipt of all documents, excluding the transfer time.

Common mistakes to avoid

Giving the child's bank account: CPF pays the child's savings into the parent's or guardian's account.
Leaving out the child's birth certificate or the guardianship papers, so the relationship can't be verified.
Signing before you are in front of the CPF officer, Mission official or notary.
Assuming the adult form's "Write to Us" upload applies — Form CPF-CA(C) doesn't offer it.
Filling in section 5 when you're completing the form at a CPF Service Centre, where it isn't needed.
Ticking more than one relationship box, or none — tick exactly one.
Correcting mistakes with fluid or tape, which makes the form void.

After you fill it out

Whose bank account does the money go to?

Yours. The form's bank-details note says to provide the applicant's (parent's or legal guardian's) bank details, and the bank statement you attach must show your full name and account number.

How is this different from Form CPF-CA?

Form CPF-CA is for an adult closing their own account. CPF-CA(C) is completed by a parent or guardian for a child, adds a section for the applicant's own particulars and relationship, pays into the parent's account, and has no "Write to Us" upload option.

Can a legal guardian apply instead of a parent?

Yes. Tick "Legal guardian" and attach the legal documentation of guardianship, including any relevant court order, with your own identification documents.

Does the form have to be witnessed if we're already overseas?

Yes. It must be witnessed and certified by an official at a Singapore Overseas Mission, or by a Notary Public followed by an apostille if your country is part of the Apostille Convention. CPF may reject applications that aren't certified as it requires.

What happens to my child's MediShield Life cover?

By signing, you authorise CPF to end it once the application is approved, with unused premiums returned to the premium payer's CPF savings. If you want the cover to continue, contact CPF before submitting.

Do both parents need to sign?

The form has a single applicant: one parent or legal guardian fills in section 2 and signs as the applicant.
