What is PPS-HEUR1 household employer registration?
Under R.A. 10361, the Batas Kasambahay, people who employ a household worker (kasambahay) register as household employers with the three social security agencies: Pag-IBIG Fund, PhilHealth and SSS. The PPS-HEUR1 form lets you do that once, on a single page, instead of filling in three separate registrations.
Part I asks for any household employer numbers you already have. Part II-A is your personal information: name, date of birth, sex, TIN, address, telephone, mobile, email and the number of kasambahay you employ. Part II-B is your certification. Parts III and IV are completed by the agency that receives the form.
FileIt asks the questions in English, prints your answers in capital letters into the comb boxes of the agencies' own PDF, and saves it in your vault. Each kasambahay is then registered on the companion PPS-KUR form.

How to fill out PPS-HEUR1 household employer registration, step by step
1 Part I — Numbers already issued
If you are already registered as a household employer with any of the three agencies, enter that number in its box: Pag-IBIG household employer number or RTN, PhilHealth Employer Number, or SSS household employer ID. Leave the boxes blank for a first registration.
2 Part II-A — Your name, birth date and TIN
Write your last name, first name, name extension and middle name, or tick the box if you have no middle name. Your date of birth goes in month-day-year boxes and your TIN, if any, in the 3-3-3 boxes. Tick male or female.
3 Address and contact details
Give your address in the order printed: unit, building, lot/block/house number, street, subdivision, then barangay, municipality or city, province or region and ZIP code. Then your telephone (10 boxes with the area code), mobile number, email and the number of kasambahay you employ.
4 Part II-B — Certification
You certify that the information is true and correct for your registration with Pag-IBIG, PhilHealth and SSS. FileIt prints your name under the signature line and the date; you sign over your printed name.
Common mistakes to avoid
- Writing in lower case — the form asks for capital letters (FileIt prints them that way).
- Leaving the middle name blank without ticking 'no middle name'.
- Filing through someone else without the letter of authorization and both valid IDs.
- Registering yourself but forgetting the separate PPS-KUR registration for each kasambahay.
After you fill it out
Print the form from your FileIt vault and sign over your printed name.
Hand it in at Pag-IBIG, PhilHealth or SSS, yourself or through an authorized representative with the documents the instructions list. FileIt does not submit it for you.
Keep the PDF in your vault with your kasambahay's PPS-KUR form and their employment records.
