What is IND 7645 authorisation declaration?

Form 7645, the IND's Authorisation declaration, lets you authorise another person to deal with the Immigration and Naturalisation Service (IND) on your behalf. You fill in your own details, the details of the person or company you authorise, and tick exactly what they may do: submit an application, a request or a notification, file an objection, a complaint or a notice of default, or something else you describe.

The form also covers collecting a residence document or residence endorsement sticker, but only when your medical situation keeps you from travelling to the IND desk for a long time, even accompanied; the representative then brings a doctor's note as well. Is the other person your lawyer? Then the form says no authorisation is needed.

FileIt shows every question in plain English, prints your answers in block letters into the boxes of the IND's own PDF, and saves the finished form in your vault. You sign the printout by hand and enclose a copy of your passport or ID card. FileIt never sends anything to the IND.

How to fill out IND 7645 authorisation declaration, step by step

1 1. Details of you

Your surname and first names as in your passport, your V-number if you know it, sex, date of birth (day, month, year boxes), place and country of birth and a phone number. The form has 10 boxes for the number.

2 2. Details of the authorised representative

The company name if there is one, the representative's name, a contact person, their postal address (street and number, or P.O. box), postcode, town, phone number and email.

3 3. Declaration

Tick everything the representative may do for you. Collecting your residence document or sticker is only possible on medical grounds; 'other' lets you describe anything else on two lines.

4 4. Supporting documents

Enclose a copy of the page of your passport or ID card with your identity details, so the IND can check your signature, and a doctor's note if the representative will collect a residence document. The representative brings your original passport to the desk.

5 5. Signing

Give the place and date and sign the printout by hand on the 5.2 line.

Common mistakes to avoid

  • Forgetting the copy of your passport or ID card.
  • Ticking 'collect my residence document' without a medical reason and a doctor's note.
  • Leaving section 3 empty, so it's unclear what the representative may do.
  • Signing with a signature that doesn't match the one in your passport.

After you fill it out

Print the form, sign it and add the passport or ID copy.

If the representative will visit the IND desk, give them the declaration to bring. Otherwise enclose it with the form it is for, or post it to the IND (the address is on ind.nl/en/post).

FileIt keeps the filled PDF in your vault, so you can authorise the same person again later.
