What is 9005 request to register your departure from the EU?
If you have to leave the Netherlands, the IND doesn't always know when you actually did. Form 9005, 'Verzoek tot het registreren van feitelijk vertrek', asks the IND to record your actual departure from the territory of the European Union (without Ireland) plus Norway, Iceland, Liechtenstein and Switzerland, which the form calls 'the territory of the EU'.
The form lists when this can matter: you received a return decision and are listed in the Schengen Information System (SIS), you received an entry ban that hasn't started because the IND doesn't know you left, or you want to prevent a return decision after your residence permit was withdrawn. You give your details, passport, address abroad, the date you left and whether you have re-entered since, and you prove the dates with passport stamps or other documents. A representative can file it for you.
FileIt lets you answer in English with the Dutch question text shown, checks that the dates are consistent, and prints your answers into the IND's own PDF, including the V-number and date boxes. The finished PDF is saved in your FileIt vault. FileIt never sends the request; you post it with the evidence yourself.

How to fill out 9005 request to register your departure from the EU, step by step
1 1. Gegevens van u (your details)
V-number if known, name as in the passport, sex, date and place of birth, country of birth, nationality, passport details, your address abroad, phone and email. Dates go in the day-month-year boxes.
2 2. Gemachtigde (representative)
Switch this part on only if someone else files the request. A representative who isn't a lawyer based in the Netherlands must enclose an authorisation signed by you, as the form says.
3 3. Verzoek registratie (the request)
Give the date you left the territory of the EU, and say whether you've entered it again since; if so, give that date. Both dates need proof.
4 4. Documenten (documents)
Enclose the passport copies, and if the stamps don't show the dates, other evidence such as a ticket and boarding pass in your name, a record of border crossings, a municipal registration, an employment contract, bank statements, payslips or utility bills. Foreign official documents must be legalised and translated into Dutch, English, French or German.
5 5. Ondertekening (signature)
Give the name of the person signing, the place and the date, and sign on the line. Sign by hand on the printout.
