What is IND 7608 declaration of relationship?

Form 7608, the IND's Appendix Declaration of relationship, is only for a residence permit for residence with, or for, an unmarried partner. The sponsor (the partner in the Netherlands) and the foreign national each give their details, civil status and home address.

Together they declare that they maintain an exclusive relationship, run (or will run) a joint household, and have been (or will be) cohabiting at the sponsor's address from a given date. If you don't live together yet because your partner isn't in the Netherlands, you give the expected date of arrival. Both partners also promise to tell the IND within 4 weeks if the relationship ends, as the form says. Both sign it.

Only one partner signing.
Not telling the IND within 4 weeks when the relationship ends.
