HZMO Tiskanica 1.4 — Zahtjev za priznanje prava na obiteljsku mirovinu (Croatian survivor's pension application)

What is HZMO form 1.4 (survivor's pension)?

When an insured person or a pensioner dies in Croatia, their widow or widower, children and in some cases parents may be entitled to a survivor's pension (obiteljska mirovina) from the Hrvatski zavod za mirovinsko osiguranje (HZMO), the Croatian Pension Insurance Institute. They apply on Tiskanica 1.4 with the HZMO regional office for where they live, and several family members can claim on one form.

The nine-page form asks who applies, the deceased's details, OIB and date of death, any right HZMO had already granted them, their company and craft roles, work abroad and foreign pensions, their children, foster care and special statuses, their work and the cause of death, insurance periods still to be proved, and a table of the family members who claim, with their relationship, OIB, address and whether they work, already receive a pension or, for children over 15, are in school.

Who needs to fill out HZMO form 1.4 (survivor's pension)?

• Widows, widowers and cohabiting partners of a person who was insured in Croatia or received a Croatian pension.
• Children of the deceased, including children over 15 in full-time education and children with a disability status.
• Parents the deceased supported, and guardians or attorneys applying for them.

When to use HZMO form 1.4 (survivor's pension)

• After the death of an insured person or pension beneficiary, choosing the date from which the survivor's pension should start.
• When several family members claim together: list each of them in the family table.
• Not for the payment of part of a survivor's pension (Tiskanica 1.4a) or for an old-age or disability pension, which have their own HZMO forms.

What you need before you start

• The deceased's OIB, date and place of birth, last address, citizenship and date of death.
• Details of any HZMO decision the deceased had, and of their company roles, work abroad and foreign pensions.
• Each claiming family member's name, relationship, date of birth, OIB, citizenship and address.
• The death certificate, birth and marriage certificates, school certificates for children over 15, and, if the deceased wasn't yet a pensioner, their work booklet (radna knjižica).

How to fill out HZMO form 1.4 (survivor's pension), step by step

1 Who applies and the deceased

The HZMO office, whether you apply as widow or widower, child, parent or child with a disability status, and the deceased's name with their father's first name, OIB, dates of birth and death, sex, addresses and citizenship.

2 The deceased's rights and roles

Any right HZMO had granted, crafts and company roles, HZZ benefits, work abroad and foreign pensions.

3 Children, foster care and status

If the deceased was a mother, or a father who used most of the parental leave, their children; then foster care and special statuses such as veteran.

4 Work, cause of death and periods

The deceased's employment, last job, the cause of death, the start date, and the optional period tables.

5 Family members

One row per claiming family member, with DA or NE for work, existing pensions and school, plus remarriage and general incapacity for work.

6 Signature and contact

A guardian's details if one applies, place, date, signature and contact details.

Common mistakes to avoid

• Using the batch copy of 23 December 2025 that HZMO replaced the same day. The current file asks whether the deceased was a foster carer for more than 10 years.
• Mistyping an OIB. FileIt checks each check digit for you.
• Leaving out a family member who also claims. List every claimant in the table.
• Forgetting the marriage certificate issued after the death (not older than 6 months) for a widow or widower.

After you fill it out

Sign the form and attach the documents listed on its last page, plus the bank's Ovlaštenje/Suglasnost form for payment to an account.

File it with your HZMO regional office in person or by post. FileIt does not send it for you.

Keep the filled PDF in your FileIt vault, and file HZMO's decision beside it when it arrives. The form reminds you to report changes, such as marriage or the end of schooling, within 15 days.
