What is HZMO form 1.1 (old-age pension)?

In Croatia the first-pillar pension is paid by the Hrvatski zavod za mirovinsko osiguranje (HZMO), the Croatian Pension Insurance Institute. To claim it you file Tiskanica 1.1 with the HZMO regional office for where you live, ticking one of four pensions: the old-age pension, the early old-age pension, the pension for long-term insured persons, or the early pension after an employer's bankruptcy.

The seven-page form asks for your details and OIB, any right HZMO has already granted, company and craft roles, work abroad and foreign pensions, children (for the insurance period credited to mothers, or to fathers who took most of the parental leave), foster care, special statuses such as Homeland War veteran, your work and the date the pension should start. Tables let you list insurance periods that still need to be proved.

Who needs to fill out HZMO form 1.1 (old-age pension)?

  • People insured in Croatia who are reaching the age for an old-age or early old-age pension.
  • Long-term insured persons and workers whose employer went bankrupt, under the special early-pension routes.
  • People with insurance periods abroad or still unproven periods in Croatia, who list them in the tables.

When to use HZMO form 1.1 (old-age pension)

  • When you are ready to claim, choosing the date from which you want the pension to start.
  • After or alongside the notice of intent to claim (Tiskanica 5.1), if you sent one with your work booklet.
  • Not for a disability or survivor's pension, which have their own HZMO forms.

  • Your OIB, date and place of birth, address and citizenship.
  • Details of any HZMO decision you already have, and of company roles or crafts you hold.
  • Your children's names and OIBs, if you are the mother, or the father in the parental-leave case.

How to fill out HZMO form 1.1 (old-age pension), step by step

1 Pension type and your details

The HZMO office, the pension you claim, your name with your father's first name, OIB, date and place of birth, sex, addresses and citizenship.

2 Existing rights and roles

Any right HZMO already granted, whether you are in the crafts register or hold a company role, HZZ benefits, work abroad and foreign pensions.

3 Children

For mothers, or fathers who used most of the additional parental leave: number of children and each child's name and OIB.

4 Foster care, deferral and status

Long foster care, any EU/EEA pension you want to defer, and special statuses such as veteran or HAZU member.

5 Work and start date

Whether you still work, your last job, the start date, and whether you want the pension without stopping certain activities.

6 Periods, signature and contact

The optional period tables, a guardian's details if one applies, place, date, signature and contact details.

Common mistakes to avoid

  • Using HZMO's older fillable file of 2023. The current edition, of December 2025, adds questions on foster care and continuing activities.
  • Mistyping the OIB. FileIt checks its check digit for you.
  • Leaving DA/NE questions blank. Each one needs an answer.
  • Forgetting the attachments listed on page 7, such as the work booklet and children's birth certificates.

What if I worked abroad?

Say where you worked, list any periods you can't prove in the last table, and attach what you have. A foreign pension you already receive goes on page 2.
