What is HZMO form 1.2 (disability pension)?

In Croatia the disability pension (invalidska mirovina) is paid by the Hrvatski zavod za mirovinsko osiguranje (HZMO), the Croatian Pension Insurance Institute, after its medical experts assess your work capacity (ocjena radne sposobnosti). You apply on Tiskanica 1.2 with the HZMO regional office for where you live, ticking either the disability pension with the work capacity assessment, or the temporary disability pension.

The seven-page form asks for your details and OIB, any right HZMO has already granted, company and craft roles, employment benefits, work abroad and foreign pensions, children (for the insurance period credited to mothers, or to fathers who took most of the parental leave), foster care, special statuses such as Homeland War veteran or asbestos exposure, your work and the date the pension should start. Applicants for the temporary disability pension also say whether they were offered a job after vocational rehabilitation. Tables let you list insurance periods that still need to be proved.

Who needs to fill out HZMO form 1.2 (disability pension)?

  • People insured in Croatia whose illness or injury has reduced or ended their capacity to work.
  • People whose work capacity HZMO has to assess before a disability pension can be granted.
  • People who finished vocational rehabilitation and are still waiting for a suitable job, for the temporary disability pension.

When to use HZMO form 1.2 (disability pension)

  • When your chosen doctor (izabrani doktor) has prepared the report on your health and work capacity (form 1-IN).
  • When you want HZMO to assess your work capacity, choosing the date from which the pension should start.
  • Not for an old-age or survivor's pension, which have their own HZMO forms.

1 Right and your details

The HZMO office, whether you claim the disability pension with the work capacity assessment or the temporary disability pension, and your name with your father's first name, OIB, date and place of birth, sex, addresses and citizenship.

2 Existing rights and roles

Any right HZMO already granted, crafts and company roles, HZZ benefits, work abroad and foreign pensions, and, for the temporary pension, job offers after rehabilitation.

3 Children

For mothers, or fathers who used most of the additional parental leave: number of children and each child's name and OIB.

4 Foster care and status

Long foster care and special statuses such as veteran, asbestos exposure or HAZU member.

5 Work and start date

Whether you still work, your last job and what it involved, and the start date.

6 Periods, signature and contact

The optional period tables, a guardian's details if one applies, place, date, signature and contact details.

Common mistakes to avoid

  • Filing without the doctor's report (form 1-IN) and the job description (form 2-IN).
  • Mistyping the OIB. FileIt checks its check digit for you.
