What is Form ID 854 (amend an HKSAR passport)?
You can't change anything in an HKSAR passport by hand. When your appearance, name or another particular changes, you apply on form ID 854 and ImmD issues a new passport, valid until the expiry date of the current one.
The form asks where you will collect the new passport (an office code, or a Passport Collection Kiosk at designated offices), your particulars as stated in the passport, your HKID card, place of birth and passport details, the amendments you want, and a declaration signed by the holder, or by a parent or legal guardian if the holder is under 16.
FileIt fills the 9/2025 edition into ImmD's own PDF. It splits identity card numbers and dates into the form's boxes and checks that the right person signs for the holder's age. The finished form goes into your vault. You take it to ImmD in person.
