What is PSC04 Change of an individual PSC's details?

Once an individual is on a company's register of people with significant control (PSC), the company must keep their details up to date at Companies House. PSC04 notifies a change under section 790LD of the Companies Act 2006: a new name, a new service address or home address, a move to another country, a new nationality, or a change in how they control the company, such as a bigger shareholding.

The form starts with the PSC's details as currently registered, so Companies House can find the right record, then the date of the change. You complete only the sections that changed on that date. A change on a different date needs its own form. The October 2026 edition (Version 5.1) is the current one, and Companies House asks you to use its online service where you can.

FileIt shows each change as its own part you can switch on, fills the company number, dates and postcodes into their single-character boxes, and prints everything onto Companies House's own PSC04 PDF. It's authenticated with a printed name, not a signature. FileIt never files it for you.

How to fill out PSC04 Change of an individual PSC's details, step by step

1 Sections 1 to 3: Company, PSC and date

The company number and name, the PSC's current details as on the register, and the date the change happened.

2 Section 4: Change of name

The new title, forenames and surname.

3 Sections 5 and 5a: Addresses

A new service address for the public record, which can be the company's registered office, and a new home address on page 2, which isn't public. 'Same as service address' is allowed unless the service address is the registered office.

4 Section 6: Other details

A new country or state of residence, or a new nationality.

5 Sections 7 to 9: Nature of control

Tick every condition that now applies, new or continuing: share and voting bands, the right to appoint or remove directors, or significant influence or control, directly or through a firm or trust.

6 Sections 10 and 11: Confirmation and authentication

The date the company had confirmation of the change, then the printed name of the director, secretary or other authorised person.

Common mistakes to avoid

  • Giving the new name in section 2. Section 2 is the name as currently registered, and the new name goes in section 4.
  • Putting changes from different dates on one form.
  • Ticking 'significant influence or control' as well as a share, voting or directors statement in section 7.
  • Ticking only the new conditions in sections 7 to 9 and leaving out ones that still apply.
  • A PO Box, DX or LP number as an address.
  • Using PSC04 when the PSC has stopped being a PSC. That's PSC07.

After you fill it out

Print the PSC04 at full size on white A4 paper and check every page.

Upload it with Companies House's upload service, or post it to Companies House, Crown Way, Cardiff, CF14 3UZ. With a section 790ZF exemption, post the whole form to The Registrar of Companies, PO Box 4082, Cardiff, CF14 3WE.

Update the company's own PSC register, and keep a copy in the company's folder in FileIt.

PSC04 Change of an individual PSC's details: frequently asked questions

Which details does PSC04 change?

An individual PSC's name, service address, usual residential address, country or state of residence, nationality and nature of control. Each has its own section, and you only complete the ones that changed.

Is the new home address made public?

No. The usual residential address goes in section 5a on page 2, which isn't shown on the public record. The service address in section 5 is public.

Does someone have to sign it?

No. Section 11 asks for a printed name only. The form says you don't need to include a signature.

What if several things changed on different days?

Use a separate PSC04 for each date of change. All changes that happened on the same date can go on one form.

Can I file the change online?

Yes. Companies House asks people to use its online service where they can, because paper forms take longer to process.
