What is PSC01 Individual person with significant control?

A person with significant control (PSC) is someone who owns or controls a company, for example by holding more than 25% of its shares or voting rights, having the right to appoint or remove a majority of the board, or otherwise having significant influence or control. Form PSC01 tells Companies House about a new individual PSC, under section 790LA of the Companies Act 2006.

The August 2026 edition (Version 5.0) also asks for the PSC's identity verification: a statement that they have verified their identity with Companies House and their 11-character Companies House personal code. Companies House asks you to use its online service where you can, because paper forms take longer to process.

Who needs to fill out PSC01 Individual person with significant control?

A UK company that has a new individual person with significant control, such as a shareholder with more than 25% of the shares or votes.
A company whose PSC controls it through a firm (such as a partnership) or a trust. Sections 6 and 7 cover those.
Not for a relevant legal entity (use PSC02) or an other registrable person (use PSC03).
Not when the individual is applying, or has applied, for protection from having their details disclosed on the public register. Companies House says to email [email protected] for the correct form.

When to use PSC01 Individual person with significant control

When someone becomes a person with significant control of the company, once the company has confirmed their status and all their required particulars.
When you are notifying the register on paper instead of through Companies House's online service.

What you need before you start

The company number and exact registered name.
The date the individual became a registrable person (not before 6 April 2016).
The PSC's title, full forenames, surname, country or state of residence, nationality and full date of birth.
A service address for the public record, and their usual residential address. Neither can be a PO Box, DX or LP number.
How they control the company: the share and voting bands, any right to appoint or remove directors, or significant influence or control. Also whether that is directly, through a firm, or through a trust.
If they have verified their identity: their 11-character Companies House personal code.
The date the company had confirmation of their status and particulars, and the printed name of the person authenticating the form.

How to fill out PSC01 Individual person with significant control, step by step

1 Sections 1 and 2: Company and date

The company number and name, then the date the individual became a registrable person. This can't be before 6 April 2016.

2 Section 3: Individual's details

Title (optional), full forenames, surname, country or state of residence, nationality and month and year of birth. Nationalities and countries must come from Companies House's standard list.

3 Section 4: Service address

The address that appears on the public record. It doesn't have to be the person's home. If you give their home address here, it will be public.

4 Sections A1 to A3: Private details

The full date of birth and usual residential address, on a page not shown on the public record. You can write 'Same as service address' unless section 4 says 'The Company's Registered Office'. Tick A3 only if the individual is applying for, or has been granted, a section 790ZF exemption. The form then goes to a different postal address.

5 Sections A4 and A5: Identity verification

Only if the PSC has verified their identity and shared their personal code with you. Tick the statement, enter the 11-character code, and say why if the name on the form doesn't match their verified name.

6 Sections 5 to 7: Nature of control

Tick each that applies: the share band, the voting band, the right to appoint or remove a majority of the board, and significant influence or control (only if none of the others apply). Sections 6 and 7 are for control through a firm or a trust.

7 Sections 8 and 9: Confirmation and authentication

The date the company had confirmation of the person's status and all their required particulars, whichever is later. Then the printed name of the director, secretary or other authorised person. No signature is needed.

Common mistakes to avoid

Using PSC01 for a company or other legal entity that controls the company. That's PSC02.
Giving a home address as the service address without meaning to. It will appear on the public record.
Writing 'Same as service address' for the home address when the service address is the company's registered office.
A PO Box, DX or LP number as an address.
Ticking 'significant influence or control' as well as a share, voting or directors statement in section 5.
A nationality or country that isn't on Companies House's standard list.
Sending the form before the company has confirmation from the PSC that their particulars are correct.

What counts as significant control?

The form's statements are: holding more than 25% of the shares, holding more than 25% of the voting rights, holding the right to appoint or remove a majority of the board, or otherwise having the right to exercise, or actually exercising, significant influence or control. The same tests can apply through a firm or a trust.

Is the PSC's home address made public?

No. The usual residential address and the full date of birth go on page 2, which isn't shown on the public record. The service address in section 4, the name, nationality, country of residence and month and year of birth are public.

Does someone need to sign the PSC01?

No. Section 9 asks for a printed name only. The form says you don't need to include a signature.

What is the Companies House personal code?

An 11-character code a person gets after verifying their identity with Companies House. Only fill in sections A4 and A5 if the PSC has verified their identity and shared their code with you.

Where do I send the PSC01?

To Companies House, Crown Way, Cardiff, CF14 3UZ, for companies registered in England and Wales, Scotland and Northern Ireland alike, or through the upload service. With a section 790ZF exemption, post it to The Registrar of Companies, PO Box 4082, Cardiff, CF14 3WE.

Can I file it online instead?

Yes. Companies House asks people to use its online service where they can, because paper forms take longer to process.
