CH01 Form: Change of Director's Details (PDF)

What is CH01 Change of director's details?

Form CH01 tells Companies House that a company director's personal details have changed. It covers a change of name (after marriage, say), a new service address (the correspondence address on the public register), a new usual residential (home) address, a new country or state of residence, and a change of nationality. It is only for directors who are people. A corporate director's changes go on form CH02.

You identify the director as they're currently registered, give the date of the change, then fill in only the parts that changed. The home address page is kept off the public record. Everything else on the form is public. A printed name authenticates it, with no signature.

Who needs to fill out CH01 Change of director's details?

A company whose director has changed their name, for example after marriage or a civil partnership.
A director who wants a different service address on the public register, such as the company's registered office.
A director who has moved home.
A director who now lives in a different country, or who has a new or additional nationality.
Accountants and company secretarial agents keeping a client's register up to date.
Not a corporate director (use CH02) and not a company secretary, who has separate forms.

When to use CH01 Change of director's details

After any change to a director's registered details, giving the date the change happened.
When you can't or would rather not use Companies House's online service. Companies in the PROOF scheme must file online.
One form can record several changes for the same director, as long as they share the same date of change.

What you need before you start

The company number and name exactly as on the register.
The director's name as currently registered, and optionally their month and year of birth.
The date the details changed.
The new name, service address, home address, country or state of residence, or nationality. Only what changed.
For nationality and country, the wording from Companies House's standard list (linked from the CH01 page on gov.uk).
The name of the person authenticating the form.

How to fill out CH01 Change of director's details, step by step

1 Company details and the director's current details

Enter the company number and name, then the director as they are on the register now: month and year of birth (voluntary), title, full forenames and surname.

2 Date of change

The date the new details took effect. Then fill in only the sections for what changed.

3 Change of name (section 4)

The director's new title, forenames and surname.

4 Change of service address (section 5)

The address that will appear on the public record. It doesn't have to be the home address. You can write 'The Company's registered office' if it's the same. It must be a physical address, not a PO Box (unless part of a full service address), DX or LP number. Tick the box confirming the home address hasn't changed, or complete section 5a.

5 Change of usual residential address (section 5a)

The director's new home address. This page isn't shown on the public record. You can write 'Same as service address', unless the service address is the registered office. It can't be a PO Box, DX or LP number.

6 Change of other details (section 6) and authentication

A new country or state of usual residence, or a new nationality. Then the person authenticating prints their name. A director, the secretary or another person the form lists can do this, and no signature is needed.

Common mistakes to avoid

Putting the director's new name in section 2. Section 2 is the name currently on the register; the new name goes in section 4.
Changing the service address without either ticking the 'no change' box or giving the new home address in section 5a.
Using a PO Box, DX or LP number as a home address.
Writing 'Same as service address' for the home address when the service address is the registered office.
Nationality or country wording that isn't on Companies House's standard list.
Using CH01 for a corporate director (use CH02).

After you fill it out

Print the form and check it, then upload or post it to Companies House (see gov.uk/companies-house/offices for where to send it). FileIt doesn't send it for you.

Is my home address made public on a CH01?

No. The usual residential address goes on a separate page that isn't shown on the public record. The service address in section 5 is public, so if you put your home address there it will appear.

Can I use the registered office as my service address?

Yes. The form lets you write 'The Company's registered office' as the service address.

Does CH01 cover a corporate director?

No. CH01 is for directors who are individuals. A corporate director's details are changed with form CH02.

Can I change more than one thing on one CH01?

Yes. Complete each section that applies. They all share the one date of change in section 3.

Does CH01 need a signature?

No. On this edition the person authenticating the form prints their name. No signature is needed.

Is there an occupation box on the current CH01?

Not on this edition (Version 8.0, 11/25). Its 'other details' section covers only country or state of residence and nationality.
