Form MEU1B — Application for a Registration Certificate for EU Citizens in Cyprus

What is Form MEU1B (EU citizen registration, 'yellow slip')?

Form MEU1B is the Application for Issue of Registration Certificate of Union Citizen and His/Her Family Members also EU Citizens (Αίτηση για έκδοση βεβαίωσης εγγραφής πολίτη της Ένωσης). It is how a citizen of the European Union, the EEA or Switzerland who stays in Cyprus for longer than three months applies for the registration certificate widely known as the 'yellow slip' (MEU1). Spouses and children who are also EU citizens are listed on the same form.

The form's own instructions say it is submitted within four months of arriving in Cyprus, with every field completed, the supporting documents for your case and the fee printed on the form (for this edition, €20 for the EU citizen and €20 for each family member's application). It is handed in in person, because a photograph is taken when you submit it: at the central offices in Nicosia, or at the district units of the Police Aliens and Immigration Service for the other districts. The form still carries the name of the Civil Registry and Migration Department; the Migration Department now publishes it.

• EU, EEA and Swiss citizens who move to Cyprus to work for an employer.
• EU citizens who are self-employed in Cyprus.
• EU citizens living in Cyprus on their own resources, such as a pension, savings, rent or dividends.
• Spouses and children of an EU citizen who are themselves EU citizens and apply at the same time.
• Anyone who has stayed or plans to stay in Cyprus for longer than three months and needs proof of registration.

When to use Form MEU1B (EU citizen registration, 'yellow slip')

• Within four months of the date you arrived in Cyprus, as the form's instructions require.
• Once you have a residence address in Cyprus and the supporting documents for your situation (employment, self-employment or own resources).
• When a spouse or child who is also an EU citizen joins you, listing them in Part II.
• A family member who is not an EU citizen doesn't use this form; the Department has a separate application for them.

What you need before you start

• A valid passport or national identity card, with its number and expiry date.
• Your date and place of birth, nationality, maiden surname if any, and the date you last entered Cyprus.
• Marriage or civil partnership and divorce dates, if they apply.
• Your residence address in Cyprus and abroad, phone numbers and email, plus the separate Declaration of Mailing Address form.
• For family members: full name, nationality, date and place of birth, relationship, country of residence, ARC if any, and travel document number and expiry.
• If employed: the employer's name, registration and VAT numbers, address and contacts, your monthly salary, social insurance number, position and contract length, and the employer's signature and stamp.
• If self-employed: your registration number, professional licence, VAT number, declared monthly salary and Social Insurance registration number.
• If living on other income: the source, bank or company and country, and the amount of each income and allowance.
• The supporting documents on the Department's list, in Greek or English or officially translated, and duly certified (apostille) where issued abroad.

How to fill out Form MEU1B (EU citizen registration, 'yellow slip'), step by step

1 Part I: Particulars of the Union citizen

Give your surname, names, maiden surname, date of birth, nationality and place of birth (city and country), then your passport or identity card number and its expiry date, and the date you last entered Cyprus. Tick your marital status and sex, and add the dates of marriage or civil partnership, divorce application and divorce decision where they apply.

2 Addresses and contact details

Give your residence address in Cyprus (street and number, area, city and four-digit postal code) and your residence address abroad. The form asks you to attach the Declaration of Mailing Address form. Then give your home, office and mobile numbers, fax and email, and a postal address, or 'Same as above' if it is your residence address. The declaration in Part VI covers your address, phone number and email too, so make sure they are current.

3 Part II: Family members (spouse and children)

List your spouse and children who are also EU citizens and apply with you, one row each: full name, nationality, date and place of birth, relationship to you, sex, country of residence, ARC number if they have one, and travel document number and expiry date. The table has five rows. A spouse listed here also signs the declaration in Part VI.

4 Part III: Particulars of the employment

For employees. Give the company or employer's name, its employer's registration and VAT numbers, the address of employment and the employer's contact details, the sector of activity, your monthly salary or income in euro and your own social insurance number, the position offered and the expected length of the contract. The form notes that a certificate of registration from the Social Insurance Services is needed.

The employer signs, dates and stamps this part by hand.

5 Part IV: Declaration of self-employment

For the self-employed. Describe the type of service and sector, and give your individual or legal entity registration number, professional licence, VAT number, declared monthly salary and Social Insurance Services registration number, plus the address and email of your business. The form asks for a certificate of contributions for the last four months and the registration receipt.

6 Part V: Other incomes and allowances

Only for income that doesn't come from the work declared in Parts III and IV: salary from abroad, pension, deposit interest, shares or rents, naming the bank, company or type of property, the financial institution and country, and the amount in euro. Allowances go in the lower table with their country. FileIt adds up both totals.

7 Part VI: Official declaration

The EU citizen, and the spouse if included, declare that everything in the application and the documents attached is true and correct, and accept that the Department may re-examine the application under article 27(2) of Law 7(I)/2007 and cancel the residence permit if that isn't possible. Common mistakes to avoid

• Applying after the four-month deadline printed in the instructions.
• Leaving fields blank: the instructions say every field must be completed.
• Forgetting the Declaration of Mailing Address form that goes with the residence address.
• Submitting foreign documents without an apostille or official translation into Greek or English.
• Handing in Part III without the employer's signature, date and stamp.
• Listing a spouse in Part II but not having them sign Part VI.
• Using this form for a family member who isn't an EU citizen, who needs the Department's separate application.

After you fill it out

Print the form, sign Part VI (with your spouse if they are included) and have your employer sign and stamp Part III if you are employed. Take it in person with your passport or ID card, the supporting documents and the fee to the Migration Department's central offices in Nicosia or to the Police Aliens and Immigration Service unit for your district. Everyone applying needs to be present, because a photograph is taken when the application is submitted.
