What is T1158?
T1158, Registration of Family Support Payments, is the Canada Revenue Agency (CRA) form for registering a court order or written agreement for family support. The current edition is T1158 E (25).
You use it when your order or agreement was made after April 1997 and specifies support for a spouse or common-law partner, or when it was made before May 1997, specifies spousal support (or separate amounts for a spouse and a child), and either Form T1157 was filed or the child support was changed after April 1997. It records both people's details, the start and end dates and the annual amounts of child and spousal support, and whether the amounts will be adjusted.
FileIt fills in T1158 from a guided web form and prints your answers on the CRA's own PDF, including the comb boxes for SINs and dates. The finished PDF is filed in your vault. FileIt never sends anything to the CRA: you mail the form with a copy of the order or agreement yourself.
