What is Executor/Administrator request for information (SS524)?

Form SS524 lets the person with legal authority over a deceased estate ask Services Australia (Centrelink) for the deceased person's information. That can include the amount Centrelink owes the estate, what the person owed at the time of death, and what Centrelink will claim against the estate. It also covers outstanding reviews or claims, payment statements for the current and previous financial year, and statements of income and assets or Centrepay deductions at the date of death.

Services Australia only discloses this information to the executor named in the Will, the Public Trustee, a court, or the administrator of the estate. You send proof of that authority with the form: a copy of the Will, letters of administration, a court order or similar legal document, or a letter from the executor's or administrator's legal representative. If you have legal authority to receive payments owed to the estate, the form also takes the bank account, or asks for a cheque.

When to use Executor/Administrator request for information (SS524)

After Services Australia has been told of the death, usually with the Advice of death form (SA116), when the estate needs Centrelink information.
When you need to know about money owed to or by the estate, or claims Centrelink will make against it, before distributing the estate.
When you need payment statements, or a statement of income and assets at the date of death, for the estate's tax or legal work.
When you have legal authority to receive Centrelink payments owed to the estate and want to nominate the account or ask for a cheque.

The deceased person's name, date of birth, permanent address before death, date of death and relationship status, plus their Customer Reference Number if known.
Your authority: executor, administrator, Public Trustee or court, and proof of it (for example a copy of the Will, letters of administration or a court order).
Your name, postal address, daytime phone number, and your organisation name and internal reference if you act for one.
For payments owed to the estate: the bank name, BSB, account number and account name, or a request for a cheque.

1 Deceased person's details (questions 1 to 7)

Their Customer Reference Number (if known), name, date of birth, permanent address before death, date of death, and relationship status at the time of death with the partner's name if applicable. Question 7 asks under what authority you're requesting the information. Information is only released if you have the appropriate authority.

2 Authorised person's details (questions 8 to 13)

3 Payments to the estate (question 14)

Answer yes only if you have legal authority to receive payments owed to the estate. Then give the bank, building society or credit union, BSB, account number and account name, or tick for payment by cheque.

Common mistakes to avoid

Sending the form without proof of authority. Information is only released to someone with the appropriate authority.
Using SS524 to report the death. That's the Advice of death form (SA116(a) for an adult).
