What is Advice of death — adult (SA116(a))?

Form SA116(a), Advice of death – adult, is how a family member, executor, friend or organisation tells Services Australia that an adult has died. One form updates the records Services Australia keeps for Centrelink, Medicare and Child Support. Services Australia asks for it to be returned within 28 days of the death.

The form asks for the deceased person's details and reference numbers (if you know them), their home address and date of death, and their relationship status. It asks whether they received or paid child support. Where known, you give details of the hospital, funeral director, executor or administrator, and next of kin. Then you give your own details and sign the declaration. A child's death uses a separate form, SA116(b).

How to fill out Advice of death — adult (SA116(a)), step by step

1 Deceased person's details (questions 1 to 7)

Their name, date of birth, Medicare card number and reference number, Centrelink Customer Reference Number, Child Support reference number, home address and date of death. 2 Relationships and background (questions 8 to 12)

Their relationship status and partner's name. Questions 9 and 10, about Aboriginal, Torres Strait Islander and Australian South Sea Islander descent, are voluntary. Question 11 asks whether they received child support for a child in their care, or paid child support. If they did, question 12 asks where the children are living.

3 Hospital and funeral director (questions 13 to 22)

If known, the hospital's name, address, phone number and contact name, and the funeral director's business name, address, phone number and contact. Answer 'Not applicable' or 'No' to skip them.

4 Executor or administrator (questions 23 to 28)

If the executor or administrator of the estate is known, give their name, organisation, postal address and phone number. Question 28 asks whether they'll request information from Services Australia. If so, they'll need to complete an Executor/Administrator Request for information (SS524).

5 Next of kin (questions 29 to 33)

If known, the next of kin's relationship to the deceased person, name, address and phone number.

6 Your details and declaration (questions 34 to 40)

Common mistakes to avoid

  • Using SA116(a) for a child. A child's death is notified on SA116(b).
  • Leaving it too long. Services Australia asks for the form within 28 days of the death.
  • Assuming this form gets the executor the deceased person's information. That needs a written request or an SS524.
  • Forgetting to sign the declaration after printing.
  • Leaving out the child support question when the person received or paid child support.
