What is the traffic offense intake form template?

Traffic and driving cases move fast. A response deadline on a ticket or a first court date is often only weeks away, and the outcome can turn on details — how a speed was measured, what test was given, how many points are already on the licence, whether the client drives for work.

This three-page intake collects them. The client gives their contact details, date of birth, where their licence was issued and what kind they hold; commercial licence holders see a note, and everyone is asked whether they drive for work. The offence page asks the charge, the wording on the paperwork, ticket number, date, time and location, with follow-ups for speeding (speed, limit and how it was detected) and drink or drug driving (sample type and reading).

Accident, injuries, arrest and on-the-spot suspension come next, then the client's own account. The final page covers points, prior offences, the court and date, what the client is thinking of doing, hardship and uploads. Each intake is saved as a PDF in your FileIt vault.

The traffic offense intake form, page by page

1 About you

The client enters their name, Email, Phone and Date of birth, where the licence was issued and which licences they hold — car, motorcycle, commercial, learner, or none. Commercial holders see a short note. Do you need to drive for work? is required.

2 The offence

What is the charge? lists speeding, drink or drug driving, careless or reckless driving, no insurance, driving while disqualified, phone use, red light or stop sign, and failing to stop, plus Other. The charge wording, ticket number, date, time and location follow. Speeding asks the alleged speed and limit and how it was detected; drink or drug driving asks which sample was given and the reading. Questions about an accident, injuries, arrest and licence suspension lead into the client's own account.

3 History, court & documents

Points or demerits on the licence and previous offences in the last five years come first, with details if any. The Court and court date or response deadline follow, and What are you thinking of doing? offers contest, admit and explain, or not sure. A hardship question, uploads for the ticket and paperwork, a dashcam and witnesses question and an acknowledgement close the form.

Make the template yours

  • Rename the charge list to match the offences in your jurisdiction
  • Change points or demerits to whatever system your licensing authority uses
  • Add a question about vehicle ownership or company vehicles
  • Add a fixed-fee paragraph for common offences if you offer them
  • Notify a duty lawyer's phone by email for DUI enquiries
  • Ask for the notice of intended prosecution date if you work in England and Wales

Tips for a better traffic offense intake form

  • Check the response deadline on the paperwork first
  • Ask for photos of every page of the ticket or summons, front and back
  • Dashcam footage can be overwritten quickly — tell clients to save it now
  • Hardship details are easier to gather early than the week of the hearing
  • Don't let the client's own account replace a proper interview
