What is the proof of service record form template?
Whether papers were served properly is easy to argue about months later. Details that seemed obvious on the doorstep — the time, who answered, what they looked like — fade fast. A server's handwritten note on the back of a job sheet doesn't always survive either.
This template captures service while it's fresh. The server identifies the case by name, number and court, ticks which documents were served, and records the person or business to be served and the address. They choose the method, then give the date and time, any earlier attempts, and optional photos.
Substituted service gets the most detail. Choosing it asks for the recipient's name, their relationship to the person served, a physical description, and whether copies were also mailed and when. Personal and business service have follow-ups of their own. The record is signed on screen and filed as a dated PDF in your FileIt vault, ready to support the court's proof of service form.

The proof of service record form, page by page
1 The case and documents
"Case name", case number, court and the job or client reference, then "Which documents were served?" — summons, complaint, subpoena, hearing notice, motion or order, eviction notice or other — with a note for page counts.
2 Service
"Person or business to be served" and the address where service took place. "How were the papers served?" Substituted requires the recipient's name, relationship and description, and asks whether copies were mailed and when. Personal asks for an optional description of the person served. Business asks for the name and title of the person who accepted. Date served, time served, earlier attempts and optional photos follow.
3 Server
The server's name, phone, registration or license number and fee, a declaration that they're at least 18 and not a party, a signature and the date signed.
Make the template yours
- Add the service methods your courts recognise, such as service by publication
- Make photos required if your clients expect them
- Add a GPS or landmark note field for rural addresses
- Rename the declaration to match the wording your courts use
- Send each client's records to their own vault folder
- Notify the office manager so the court form can be prepared the same day
Tips for a better proof of service record form
- Fill it in immediately after service, before driving to the next job
- Describe the recipient by what you saw, not what you assume
- Record every attempt, including the failed ones
- Check the requirements for each method of service where the case is filed
Every response becomes a PDF in your vault
The signed record is filed as a PDF in the vault folder you choose, with photos attached, and the office is notified by email. The server receives a copy too.
The Responses table lists every job with its case number, method and date served, so you can find a record quickly or export the list to CSV for invoicing. The record supports the court's own proof of service form; it doesn't replace it.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
