What is the newborn feeding log form template?

In the first weeks, midwives, health visitors and paediatricians often ask the same questions: how many feeds, how long or how much, how many wet and dirty nappies, what colour, how the baby is sleeping and settling, and what they weighed at the last check. Tired parents rarely remember the details, so writing it down each day helps.

This template is one log per day. Parents record the baby's name and date of birth, the date and who's logging, and how the baby was fed — breast or chestfeeding, formula, both, or expressed milk. The feeds page counts feeds in 24 hours, lists each feed's time and side or amount, totals bottle volume and ticks any problems such as a sleepy baby, painful latch, vomiting, refusing feeds or choking.

The nappies and sleep page counts wet and dirty nappies, records stool and urine colour — with prompts to contact a professional for pale or blood-stained poo — plus sleep and settling. The last page records weight checks, yellow skin or eyes, temperature, questions, and when to get urgent help. Each day's log is saved as a PDF in FileIt.

The newborn feeding log form, page by page

1 Baby & day

Record the baby's name and date of birth, the date of this log and who's filling it in, and how the baby was fed today: breast or chestfeeding only, formula only, breast and bottle, or expressed milk by bottle.

2 Feeds

Enter the number of feeds in 24 hours and list each feed with its time, side or amount and length. When bottles are used, a box totals the millilitres for the day. A required checklist covers anything unusual, from a baby too sleepy to feed or a painful latch to bringing up more than small amounts, refusing feeds, choking or very short feeds — or no problems.

3 Nappies & sleep

Count wet and dirty nappies and pick the colour of the poo, from black meconium to yellow. Pale, chalky or white poo, or poo with blood, shows a prompt to contact a professional today. The colour of wet nappies, total sleep, the longest stretch and how well the baby settles complete the page.

4 Weight & notes

Say whether the baby was weighed today and, if so, the weight and who weighed them or where. Record whether the skin or the whites of the eyes look yellow — yes shows a prompt to tell a professional today — and any temperature taken, and add notes or questions. A closing message lists signs that need urgent help.

Make the template yours

  • Add your maternity unit's or health visiting team's phone number to the cover
  • Add a question for expressing sessions if parents are building a milk supply
  • Change the bottle total to ounces if that's what families use
  • Send personal links so each family's logs land in their own vault folder
  • Turn off owner notifications if parents use the log just for themselves
  • Add a photo upload for nappies or rashes if your team asks for them

Tips for a better newborn feeding log form

  • Fill it in at the end of each day while it's fresh
  • Bring the PDFs or the Responses table to weight checks
  • Count every wet and dirty nappy — the numbers matter in the first week
  • Don't wait for the log if something worries you; call for advice
  • Share logging between carers so one person isn't doing it all
  • This log is for sharing with professionals; it doesn't diagnose anything
