What is the lease assignment request form template?
When a business sells up or relocates, the tenant often wants to hand its lease to a buyer. Most commercial leases say that needs the landlord's consent, and the landlord can't decide until it knows who the incoming tenant is, whether it can pay, what it will do in the premises and what security is offered.
This lease assignment request template collects all of that in four pages. The current tenant gives the lease date, expiry, rent, any arrears and why they want to assign. The incoming tenant's page records its structure, registration, years trading, address, contact, directors, turnover, profit and references. The use and timing page covers any change of use, alterations, completion date, premium, the security on offer and the landlord's costs. Finally the accounts, a business plan for a new company, other documents, both sides' lawyers and a signed declaration.
Each request is filed as a PDF in your FileIt vault with the accounts and documents attached, ready to send to the landlord's lawyer or surveyor.

The lease assignment request form, page by page
1 Current tenant & lease
The current tenant gives the business name, contact, email, phone and premises, the lease date, expiry date and current annual rent. If rent or service charge is outstanding, they explain the amount and how it will be settled, and they give the reason for the assignment.
2 Incoming tenant
The incoming tenant's name, business structure, registration number, years trading and registered address are recorded, along with a contact, directors or owners, annual turnover, net profit and references.
3 Use, timing & terms
The form asks whether the use will stay the same, describing any new use and permissions, and whether alterations are planned. The proposed completion date and whether a premium is paid follow. Security options include a rent deposit (with amount), a personal guarantee (with the guarantor's name), a parent company guarantee or a guarantee from the outgoing tenant, and the tenant acknowledges the landlord's costs.
4 Documents & signature
The tenant uploads the incoming tenant's accounts, a business plan if it's newly formed and other documents, names both sides' lawyers, and signs a declaration that nothing is assigned until written consent and legal documents are completed.
Make the template yours
- Change the security options to match what your leases typically allow
- Add your consent fee or cost undertaking amount to the cover text
- Make accounts required if you never consider assignees without them
- Add your lawyer or surveyor to the notification list
- Add a question about rent deposit transfer if deposits are common in your portfolio
Tips for a better lease assignment request form
- Check the lease's assignment clause before reviewing; it sets the conditions you can apply
- Look at the incoming tenant's accounts against the rent, not in isolation
- Clear up any arrears before consent, not after
- Ask for a change-of-use permission early if the use is changing
- Note statutory response times that may apply where you are, and diary them
Every response becomes a PDF in your vault
Each signed request becomes a PDF in the vault folder you choose, with accounts and other documents attached. You're notified by email and the tenant can receive a copy.
Share the PDF with your lawyer or surveyor, and track requests by premises and date in the Responses table.
- Start from this template. It opens in the FileIt Forms designer — change any question, add pages, set the rules for when questions appear.
- Share it. Turn on a public link, or send it to people by email, each with their own link. They don’t need a FileIt account.
- Get the answers as PDFs. Each response is saved as a PDF in the vault folder you choose, with uploaded files attached — and listed in a Responses table you can export to CSV.
