What is a sleepover permission form?
Hosting someone else's child overnight is a kind favour, and it goes better when you know the basics in advance: when they're being collected and by whom, what they can't eat, which medicine they take at bedtime, and who to call if their parent's phone goes to voicemail at 2 a.m. A sleepover permission form gathers all of that in one place, so nobody has to remember a string of text messages.
This form names the child, the host and where the child will sleep, with arrival and pick-up times. The parent chooses whether outings are fine, stays-at-home only, or check-first, and can note screen-time rules. It records allergies, medication with dose and time, and anything else the host should know — sleepwalking, homesickness, a night light. It says who will collect the child and that nobody else may without the parent's word, and gives the host permission to get emergency treatment if the parent can't be reached. It works just as well for a scout or church group sleepover as for a birthday party at home.

Using the sleepover permission form, step by step
1 Add the host and the dates
Your name fills in as host. Add the address, your phone, the arrival and pick-up times, and the occasion if you like.
2 Say how the night will go
Choose the supervision line that fits a family home or a group, list any other adults staying, and set the rule for outings and screens.
3 Leave health and pick-up to the parent
Send the form to the parent to fill in allergies, medication, extra notes and who will collect, or complete it together if they've told you already.
4 Both sign before the stay
The parent signs on their phone, you countersign, and the signed copy sits in your FileIt vault for the night — and the next sleepover.
Tips before you send it
- Ask the parent to hand medication over in its original packaging, labelled with the child's name.
- If several children are staying, send each family its own form so every set of allergies is on record.
- Agree a time after which you'll call the parent rather than wait, for a child who gets homesick.
- For group overnights, check your organisation's own safeguarding policy too — this form doesn't replace it.
Sign it online in minutes
- Fill in the blanks. Names, dates and amounts go straight into the agreement — the preview shows exactly where.
- Name who signs. Tick “This is me” for your own part. FileIt builds the PDF with every signature, date box on the right line, and you can check or move anything before sending.
- Send it. Each signer gets a secure link and signs from any device — no account needed. The signed PDF, with an audit trail and a Certificate of Completion, is filed in your vault and emailed to everyone.
Sleepover permission form: frequently asked questions
Does the host give a child medicine with this form?
Only what the parent lists, with the dose and time. If nothing is listed, the form says the host won't give any medicine without asking first, except in an emergency on medical advice.
What if someone else turns up to collect the child?
The form names who will collect. The host won't let the child leave with anyone else unless the parent confirms it directly by phone or message.
Is this a liability waiver?
No. It's a permission and information form. For a risky activity during the stay, such as swimming, use a separate waiver or consent.
Can parents sign without a FileIt account?
Yes. They open the link on any phone or computer and sign. The signed PDF carries an audit trail and a Certificate of Completion.
