What is a child transportation consent?
Coaches driving players to away games, a nanny doing the school run, a neighbour sharing the drive to swim practice, a club minibus to a tournament: every one of these involves someone else's child in your vehicle. A short written consent tells the driver exactly what the parent has agreed to, and gives the parent comfort that the basics — seat belts, a proper car seat, no phone at the wheel — have been promised.
This form names the child, the driver or organisation and, where an organisation is transporting, the people who will actually drive. It records the vehicle, how the child will sit, which trips are covered and for how long, and whether other passengers may ride along. The driver promises a valid licence and insurance, a roadworthy vehicle, sober driving and no hand-held phone, never leaving the child alone in the car, and handing the child only to the parent or someone named in writing. The parent adds medical needs and a back-up contact, and can withdraw consent at any time.

Using the child transportation consent, step by step
1 Name the driver and the vehicle
Your name or organisation fills in. Name the people who will drive, your phone number, and the vehicle — make, colour and plate, or the minibus.
2 List the trips
Describe the trips covered, set the start date and, if you like, an end date, and choose whether other children may travel at the same time.
3 Agree the seating
Choose who provides the car seat or booster, or confirm the child is old and tall enough for an adult belt where you'll be driving.
4 Send to the parent
The parent adds the child's details, medical needs and a back-up contact, then signs. You countersign, and the signed copy is filed in your FileIt vault.
Tips before you send it
- Check your motor insurance covers carrying other people's children, especially if you're paid or driving for a club.
- Car seat rules depend on the child's age, height and weight and differ between countries and states — check the rules where you'll drive.
- Clubs often have their own transport and safeguarding policies; use this form alongside them.
- Re-send the form each season so contact numbers and medical notes stay current.

Child transportation consent: frequently asked questions
Can one form cover regular trips?
Yes. Describe the trips (for example school to swim club on Mondays and Wednesdays) and set a date range, or leave the end date empty to keep it in place until the parent withdraws it.
Who provides the car seat?
You choose: the parent, the driver, or neither because the child can legally use an adult seat belt where the trips happen.
Can the parent change their mind?
Yes. Consent ends as soon as the parent tells the driver in writing. The driver can also decline any trip they think wouldn't be safe.
Is this the same as a child travel consent letter?
No. The travel consent letter is for a child travelling abroad or away with one parent or another adult. This form covers being driven locally by someone else.
Does the parent need a FileIt account?
No. They sign from the email link on any device. The signed PDF includes an audit trail and a Certificate of Completion.
