What is a advanced skin treatment consent?
Most problems after peels and microneedling come from things the practitioner did not know about or the client did not do: a recent course of isotretinoin, a retinoid still in use, a holiday in the sun a week later. A consent form that screens properly and spells out aftercare prevents most of them.
This form records the clinic, practitioner and qualification, the client and skin type, and the treatment: a chemical peel, microneedling, radiofrequency microneedling, dermaplaning or a combination, with a short explanation of each. It covers product strength or needle depth, the areas, what the client hopes to improve, the number of sessions and spacing, and the patch test. The client confirms their health history against common contraindications, such as pregnancy, recent isotretinoin, active infection or cold sores, keloid scarring, a recent tan, blood thinners and implants for radiofrequency, and lists medicines and skincare. It also explains risks including pigment changes and rare scarring, before and aftercare, daily sun protection, results that vary, and photos.

Tips before you send it
- Ask about new medicines, skincare and sun exposure before every session, not just the first.
- Take extra care, and consider a patch test, for skin types prone to pigment changes.
- Give written aftercare instructions to take home.
- Check local rules on which treatments and needle depths aestheticians may perform.

Advanced skin treatment consent: frequently asked questions
Which treatments does the form cover?
Chemical peels, microneedling, radiofrequency microneedling, dermaplaning, or a combination. Choosing one adds a short explanation of that treatment to the form.
Does the form list contraindications?
Yes. The client confirms their health history against a list including pregnancy, recent isotretinoin, active infections and cold sores, keloid scarring, recent sun or resurfacing, blood thinners and implants near the area for radiofrequency treatments.
Does one form cover a course of treatments?
Yes. It covers each session in the planned course until the client withdraws consent. The client must tell you about changes before each session.
Does it promise results?
No. It says results build gradually, vary from person to person, and that some concerns such as deep scarring may only partly improve.
