What is a corporate wellness services agreement?
Workplace wellness only works when staff trust it. If employees suspect their manager will see who skipped yoga or what they said in a stress workshop, they stay away. A corporate wellness services agreement makes clear that participation is voluntary, that health information stays with the provider, and what the employer does and does not get to see.
This template records the services, schedule and location, an optional participant limit, and the practitioners and their qualifications, and states that sessions are education and group activity, not medical treatment. It commits the employer to voluntary participation with no penalty or reward and no use in employment decisions, and leaves incentive rules with the employer. It requires readiness questionnaires for physical sessions, keeps health data confidential with reporting limited to the level you choose, and covers data deletion and breaches. It sets premises and safety duties, what happens if someone is hurt in a session, no recording without consent, insurance, fees and payment terms, free cancellation hours, provider cancellations, notice to end and a liability limit.

Tips before you send it
- Keep reporting anonymous unless there's a real need for names. Staff are more likely to join when they know the employer won't see who attends.
- If the employer offers rewards for taking part, they should check the rules on wellness incentives where they operate. The agreement leaves that with them.
- Have Participants complete a readiness questionnaire before physical sessions. The agreement requires it and keeps the answers confidential.
- Ask for the Employee Assistance Programme details so practitioners can point people to support.

Corporate wellness services agreement: frequently asked questions
Will the employer see employees' health information?
No. Questionnaires and anything shared about health stay with the provider. The employer gets only the reporting level you choose, never in a form that identifies individuals' health information.
Can the employer require staff to attend?
Not under this agreement. Participation is voluntary, with no reward or penalty, and attendance is not used in employment decisions.
Is this medical treatment?
No. Sessions are general education and group activity. Practitioners refer anyone who needs individual help to their own doctor or another professional.
What if a session is cancelled at short notice?
The employer can cancel free up to the hours you set. Later cancellations are charged unless the provider can use the time. If the provider cancels, there's no charge and it offers a replacement or new date.
Can it be signed electronically?
Yes. E-signatures are generally valid for business service agreements like this in the US, UK, EU and Canada. FileIt keeps an audit trail with the signed PDF.
