What is Form BT-129?
Form BT-129 is the Oklahoma Tax Commission's power of attorney. It authorizes one or more individuals to act for a taxpayer before the OTC and to get the forms and documents the taxpayer could get, for the tax types and reporting periods listed: individual income, corporate income, sales, withholding, gross production, use tax or another tax you name.
The form covers individuals and businesses. A business signs through an authorized officer, member, partner or fiduciary, who prints their name and title. You can't appoint a firm as such: you name a person within it, and you can add more representatives on page 2. Filing a new BT-129 automatically revokes earlier powers of attorney for the same matters and periods unless you tick the box to keep them. For future years, the OTC recognizes the POA for three years from December 31 of the year it receives the form, and it won't process a form that says "All" periods.
FileIt guides you through each section, checks that every tax type you tick has its period dates and that they run forwards, and fills the OTC's own PDF (Revised 7-2026). The taxpayer and the first representative can sign with FileIt's eSignature or by hand. You then submit it to the OTC yourself by fax, mail, in person or through OkTAP's power of attorney portal.

- Oklahoma taxpayers who want a CPA, enrolled agent, attorney or bookkeeper to deal with the OTC for them.
- Business owners who want their accountant to handle sales tax or withholding questions and notices.
- Spouses who each want to authorize the same representative (each files a separate BT-129).
- Representatives who need a signed authorization before the OTC will discuss a client's account.
When to use Form BT-129
- When you hire a tax professional to respond to an OTC notice, audit or account question.
- When you change representatives (a new form revokes the old one for the same matters and periods unless you keep it).
- When you add a tax type or a later period to an existing representative's authority.
What you need before you start
- The taxpayer's legal or business name, SSN, FEIN or ITIN, mailing address, email and phone.
- Each tax type to cover and the first and last reporting period for each.
- Each representative's name, whether they are an attorney, tax preparer or other, mailing address, email and phone.
- For a business: the name and title of the officer, member, partner or fiduciary signing. If they aren't on the OTC's records, Form BT-191 comes first.

How to fill out Form BT-129, step by step
1 Section I: Taxpayer
Choose SSN, FEIN or ITIN and enter the number, then the legal name, mailing address, email and phone.
2 Section II: Tax types
Tick each tax type and give the period start and end dates. Use Other for a tax not listed. Tick the retention box only if you want earlier powers of attorney to stay in force.
3 Section III: Taxpayer signature
Sign and date. Someone signing for a business also prints their name and title.
4 Sections IV and IV-A: Representatives
Name each individual representative, mark attorney, tax preparer or other, and give their contact details. Only the representative in Section IV needs to sign.
Common mistakes to avoid
- Writing "All" for the periods: the OTC won't process it.
- Leaving out an end date, which limits the authority to the start date's reporting period.
- Naming a firm instead of an individual within the firm.
- Forgetting that a new BT-129 revokes earlier ones for the same matters and periods unless you tick the retention box.
- Signing for a business when you aren't listed with the OTC as an officer, member or partner, without filing Form BT-191 first.
After you fill it out
Submit the signed form by fax, mail, in person or online through OkTAP. Mailed forms can take up to 10 business days to process.
Let your representative know when it has been submitted so they can contact the OTC.
FileIt keeps the signed copy in your vault in case you need to show who you authorized and when.
