What is North Carolina MVR-4?
Form MVR-4 is the North Carolina Division of Motor Vehicles' Application for Duplicate Title. The registered owners use it when the original certificate of title was lost or never received, including when you need a duplicate to assign to a buyer, an exact duplicate as recorded, or a duplicate with a satisfied lien removed.
The form asks for the vehicle, the registered owners, the residential, mailing and vehicle-location addresses, the liens recorded on the original title, a disclosure choice, the type of duplicate, what happened to the original, and the current odometer reading. The owners sign in front of a notary. When the title was with a lienholder, or a lien is being removed, the first lienholder completes a notarized affidavit at the bottom.
The form prints a $25.50 fee and a 15-day mandatory waiting period after NCDMV receives the application. FileIt prints your answers into the official PDF, ready for the notary, and keeps a copy in your vault. It doesn't file anything with NCDMV.

How to fill out North Carolina MVR-4, step by step
1 Vehicle description
Title number, year, make, body style, series model, VIN and fuel type.
2 Registered owners and addresses
The names under First, Middle and Last (two owners print together on the line), the residential address with city, state, ZIP and county, and a mailing or vehicle-location address only if it's different.
3 Liens on the original title
Up to three liens, each with its date, lienholder and address, exactly as shown on the original title. The duplicate is issued with those liens and mailed to the first lienholder unless a lien release is submitted.
4 Check the applicable block
Choose a duplicate and assignment by the registered owner, a duplicate as recorded, or a duplicate and removal of a lien. Then say whether the original was lost or never received, and give the odometer reading.
5 Lienholder affidavit
Only when it applies: the first lienholder certifies the title was lost while in its possession (with or without the lien satisfied), never received, or surrendered to someone on payment in full, and signs in front of a notary.
Common mistakes to avoid
- Signing before you're in front of the notary.
- Leaving off a lien that's on the original title, or listing one that isn't.
- Asking for lien removal without the lienholder's certification that the lien was satisfied.
- Forgetting the fee, or expecting the title before the 15-day waiting period ends.
- Using the old title after the duplicate arrives. It's void and must go back to NCDMV.
After you fill it out
Print the PDF and have each registered owner sign in front of a notary. If the lienholder affidavit applies, the lienholder's agent signs it in front of a notary too.
Mail it with the $25.50 fee to NCDMV, 3148 Mail Service Center, Raleigh, NC 27697-3148. FileIt keeps a copy in your vault.
