What is Form DL-14A?

Form DL-14A is the Texas Department of Public Safety application for a driver license or identification card for adults, meaning applicants 17 years 10 months of age and older. The same form covers an original card, a renewal, a replacement, a modification and an address or name change, and the license classes A, B and C with or without a motorcycle endorsement.

Page 1 collects who you are (names, Social Security number, date of birth, sex, height, weight, eye and hair color, race, ethnicity, place of birth and your parents' names), your residence and mailing addresses and phones, optional emergency contacts, and the questions every applicant answers: citizenship and voter registration, veteran fee waiver and designators, a health condition that may impede communication with a peace officer, organ donation, and six optional donations. Page 2 is for license applicants: medical history questions, and for first-time Texas license applicants, driving history and vehicle registration and insurance questions. It ends with a certification of your residence type.

FileIt shows only the questions that apply to what you're applying for, places your Social Security number between the dashes printed on the form, and prints everything on DPS's own PDF (DL-14A, Rev. 8/2025). The form says an original ink signature is required and not to sign until a notary or driver license employee tells you to, so FileIt leaves the signature blank and doesn't offer eSignature. You bring the printed application to the office yourself.

How to fill out Form DL-14A, step by step

1 What you're applying for

Choose a driver license or an identification card, then original, renewal, replacement, modify, or address or name change. License applicants pick a class (C is the standard one) and say whether they want the motorcycle endorsement.

2 Applicant information

Enter your names, Social Security number, date of birth, sex, height, weight, eye and hair color, race and ethnicity, place of birth, and your father's last name and mother's maiden name.

3 Contact information

Give your residence address with county, a mailing address if different, phone numbers and email, and up to two emergency contacts if you want to. An alternate address is only for authorized personnel.

4 Questions for everyone

Answer citizenship and voter registration, the veteran questions and designators, the peace officer communication question and organ donation. You can add any of the six optional donations in whole dollars.

5 Medical history and driving history

Driver license applicants answer seven medical questions and explain any yes. First-time Texas license applicants also answer the driving history and vehicle registration and insurance questions.

6 Certification

Choose your residence type (single family dwelling, apartment, motel or temporary shelter). Sign in ink at the office only when you're told to.

Common mistakes to avoid

  • Signing before the driver license employee or notary tells you to, or using an electronic signature: the form requires an original ink signature.
  • Using DL-14A when you're younger than 17 years 10 months.
  • Giving a P.O. box as your residence address.
  • Answering the voter registration question as a non-citizen.
  • Forgetting to explain a yes answer to the medical questions.
  • Asking for a Veteran designator or fee waiver without the proof of service or disability the form mentions.

After you fill it out

Print the application from FileIt and take it to your driver license office with the documents DPS asks for. Sign it there in ink when instructed.

If you answered yes to the peace officer communication question, the form says a physician must complete Form DL-101.

Once your new card arrives, scan it into FileIt: the expiry date goes on your reminder schedule so your next renewal doesn't sneak up on you.
