What is FK 7206 - Employer statement for pregnancy benefit (omplacering)?

In Sweden, an employee who can't keep doing her job late in pregnancy may get pregnancy benefit (graviditetspenning) from Försäkringskassan, the Swedish Social Insurance Agency. Försäkringskassan first needs to know whether her employer can reassign her. By law, a pregnant employee has the right to other duties in two situations: when her work is physically demanding and she can't continue it because of the pregnancy, or when she has been prohibited from continuing her usual work under the Work Environment Act. FK 7206 is the statement in which the employer answers.

The statement covers how she would have worked if she hadn't applied, and whether she can be reassigned: from which date, for whole days, part days or some days a week, and to which duties. If she can't be reassigned, it asks why not. Then comes either the work environment prohibition (part 5) or a detailed description of the physically demanding tasks and what you've done to ease them (part 6). The responsible manager or the person responsible for HR fills it in and signs it.

FileIt asks each question in English with the Swedish wording beside it. It pre-fills the employee's name and your company from your vault, and only shows the parts that apply. It checks the personal identity and organisation numbers and makes sure your descriptions fit the boxes. Your answers are printed into Försäkringskassan's own PDF and saved in your vault. You sign by hand and post it. FileIt never sends it.

How to fill out FK 7206 - Employer statement for pregnancy benefit (omplacering), step by step

1 1-2. The employee and the employer

Her name and personal identity number, then your company name and organisation number. FileIt also prints her personal identity number at the top of pages 2 and 3.

2 3. How she would have worked

Tick one option: according to her employment contract, in another way (say how), or not at all (say why). This is the extent she would have worked during the period the application covers.

3 4. Reassignment

Yes: the date it starts, the extent (whole days, part of the day, some whole working days a week with the number, or another split) and a description of her duties after the reassignment. No: explain why she can't be reassigned, what you've done to try, and why there are no other duties she can do.

4 5. Work environment prohibition

Tick that she is prohibited from continuing her work under a regulation issued under chapter 4, section 6 of the Work Environment Act, give the date it applies from, and describe the harmful tasks, substances or conditions behind it.

5 6. Physically demanding duties

Describe her duties and the situations that can come up in a working day, the physically demanding parts, and how you've tried to ease things. Then answer whether the duties can be changed for part of the day (and by how many hours), whether help from colleagues or work aids lets her avoid the demanding tasks, and whether she works alone or alongside others doing the same tasks. Each description has a box to tick if you give more detail in an attachment.

6 7-8. Attachments and signature

Tick the attachments you send, such as the documents behind a prohibition (a risk assessment or noise measurement) or anything else. Then add the date, phone and name in print. The responsible manager or HR person signs by hand.

Common mistakes to avoid

  • Skipping part 4. Försäkringskassan needs it whatever the reason for the claim.
  • Filling in neither part 5 nor part 6.
  • Describing the duties too briefly. The guidance asks for as much detail as possible, including the situations that can come up during a working day.
  • Saying she can't be reassigned without explaining what you tried and why no other duties exist.
  • Having someone other than the responsible manager or HR person sign.
  • Forgetting the attachments you said you would send, such as the risk assessment.

After you fill it out

Print the statement and have the responsible manager or HR person sign it in the Namnteckning box.

Add the attachments you ticked in parts 6 and 7.

Post it to Försäkringskassans inläsningscentral, 839 88 Östersund, the address printed on the form. FileIt doesn't send it.

Keep the filled copy in your FileIt vault with the employee's records.
